摘要
目的探讨生物胶体分散剂联合软聚硅酮敷料治疗临床Ⅲ期压疮合并感染患者创面愈合及肿瘤坏死因子-α(TNF-α)及血管内皮生长因子(VEGF)的影响。方法选择遵义医科大学附属医院外科2015年11月-2018年12月收治的Ⅲ期压疮合并感染患者40例作为研究对象,按照入院顺序采用随机数表法将其分为研究组(n=20)及对照组(n=20)。对患者坏死组织创面进行清除后,对照组采用重组人表皮生长因子凝胶+莫匹罗星+无菌纱布进行覆盖治疗,研究组采用生物胶体分散剂联合软聚硅酮敷料,比较两组临床疗效、治疗效果。采用酶联免疫吸附法检测两组患者治疗前后TNF-α、VEGF水平,对患者治疗前后生活质量(SF-36)评分进行统计。结果研究组治疗总有效率95.00%,高于对照组65.00%,差异具有统计学意义(P<0.05);研究组换药间隔长于对照组,换药次数、痊愈时间少于对照组,差异具有统计学意义(P<0.05);研究组治疗后压疮愈合计分量表(PUSH)、TNF-α、VEGF水平低于对照组,差异具有统计学意义(P<0.05);治疗7 d后研究组生理机能、生理职能、躯体疼痛、社会功能评分优于对照组,差异具有统计学意义(P<0.05)。结论生物胶体分散剂联合软聚硅酮敷料治疗临床Ⅲ期压疮并感染患者可明显提升患者治疗效果,加速创面愈合,减少炎症反应,值得临床推广。
OBJECTIVE To explore the effect of biocolloid dispersant combined with soft silicone dressing on wound healing and tumor necrosis factor-α(TNF-α) and vascular endothelial growth factor(VEGF) in patients with clinical stage III pressure ulcers and infection. METHODS Total of 40 patients with stage III pressure ulcers who were admitted to the Department of Surgery of the Affiliated Hospital of Zunyi Medical University from Nov. 2015 to Dec. 2018 were recruited as the research sujects. They were numbered according to the admission order, and divided into the study group(n=20) and control group(n=20) by using random number table. After the necrotic tissue wound was removed, the control group was treated with recombinant human epidermal growth factor gel + mupirocin + sterile gauze for covering, and the study group used biocolloid dispersion combined with soft silicone dressings. The clinical Curative effect and therapeutic effect between two groups were compared. Enzyme-linked immunosorbent assay was used to detect the levels of TNF-α and VEGF before and after treatment in the two groups, and the quality of life(SF-36) scores of the patients before and after treatment were recorded. RESULTS The total effective rate of treatment in the study group was 95.00%, which was significantly higher than that of 65.00% in the control group(P<0.05). The interval of dressing change in the study group was longer than that in the control group, and the times of dressing change and healing time were significantly less than those in the control group(P<0.05). The pressure ulcer healing scale(PUSH), TNF-α and VEGF levels in the study group after treatment were significantly lower than those in the control group(P<0.05). The physiological function, physical pain and social function scores in the study group after 7 days of treatment were significantly better than those in the control group(P<0.05).
