摘要
目的:观察在精准穿刺调控下经皮椎体成形术(PVP)治疗高龄(年龄≥80岁)重度胸腰椎压缩性骨折患者的临床疗效及安全性。方法:回顾分析本科室2018年1月~2019年12月对17例高龄重度胸腰椎压缩性骨折患者共22椎在精准穿刺调控下行PVP术,男4例,女13例,年龄81~98,平均(86±4.06)岁。观察术前及术后2 d、末次随访时患者VAS及ODI评分,X线或CT检查观察骨水泥渗漏及在椎体的分布情况。结果:均在精准穿刺调控下采用单侧或双侧穿刺技术,安全的完成手术。均获得3个月以上随访,术后2 d及末次随访VAS及ODI评分均较术前明显改善,差异有统计学意义(P<0.05)。本组病例出现轻度邻近椎间盘及患椎旁骨水泥渗漏5椎体。术后观察随访时均未发现任何因为骨水泥渗漏所导致的相关并发症。结论:在精准穿刺调控下PVP术治疗高龄重度胸腰椎压缩性骨折能早期快速有效缓解疼痛,疗效好,安全性高。
Objective:To observe the clinical efficacy and safety of percutaneous vertebroplasty(PVP)under precise puncture control in the treatment of severe thoracolumbar vertebral compression fractures in elderly patients(age≥80 years).Methods:from January 2018 to December 2019,PVP was performed on 22 vertebrae of 17 elderly patients with severe thoracolumbar vertebral compression fractures under precise puncture control.There were 13 females and 4 males,aged from 81 to 98 years old,averaging(86±4.06)years.The vas and ODI scores were observed before and 2 days after operation and at the last follow⁃up.The leakage of bone cement and its distribution in vertebral body were observed by X⁃ray or CT.Result:Under the control of precise puncture,unilateral or bilateral puncture technique was used to complete the operation safely.All patients were followed up for more than 3 months.The vas and ODI scores at 2 days after operation and at the last follow⁃up were significantly improved compared with those before operation(P<0.05).In this group,there were 5 vertebral bodies with slight leakage of bone cement adjacent to intervertebral disc and adjacent vertebrae.No complications were found during the follow⁃up.Conclusion:PVP under precise puncture control in the treatment of elderly patients with severe thoracolumbar vertebral compression fractures can quickly and effectively relieve pain in the early stage,with good curative effect and high safety.
