摘要
目的报道1例锥形束乳腺CT(CBBCT)引导下微波消融(MWA)治疗乳腺结节状腺病患者,结合复习文献初步观察CBBCT引导下MWA的有效性及安全性。方法患者女,60岁,左乳溢液3年余;超声示左乳3点方向距乳头约3 cm处1.7 cm×0.6 cm×0.9 cm低回声结节;穿刺病理诊断乳腺结节状腺病。行CBBCT引导下MWA,观察治疗效果及不良反应。结果顺利完成手术。术后即刻消融区呈稍高密度,完全覆盖病灶;术后3天增强CBBCT扫描显示消融区无强化。术中患者感轻度刺痛,术后2天消融侧乳腺区胀痛,未见严重不良反应。结论本例以CBBCT引导下MWA成功治疗乳腺结节状腺病,提示此法具有安全、有效治疗乳腺良性肿瘤的潜能。
Objective To report a case of cone-beam breast CT(CBBCT) guided microwave ablation(MMA) in treatment of breast nodular adenosis, and to evaluate preliminarily the effectiveness and safety of this method combining with literature review. Methods One 60-year-old female patient complained breast discharge for more than 3 years. Ultrasound examination showed a hypoechoic nodule with the size of 1.7 cm×0.6 cm×0.9 cm at 3 o’clock in the left breast, about 3 cm from the nipple. Pathological diagnosis through needle biopsy was breast nodular adenosis. CBBCT-guided MWA treatment was performed, and the therapeutic effect and complications were observed. Results The operation was successfully completed. Immediate postoperative scanning showed that the lesion was completely covered by slightly high density in the ablation, while no enhancement was found 3 days later. The patient had mild tingling pain during treatment, and distension pain in the ablation side breast 2 days after ablation, without any serious complication. Conclusion The patient of breast nodular adenosis was successfully treated with CBBCT-guided MWA, indicated that this method might had the potential to treat benign breast tumors safely and effectively.
