摘要
目的探讨无框架立体定向手术治疗儿童脑性瘫痪的安全性与有效性。方法回顾性分析109例痉挛型儿童脑性瘫痪病儿的临床资料,均应用机器人辅助无框架立体定向核团毁损手术治疗;采用粗大运动功能测试量表(gross motor function measure-88,GMFM-88)分别评估病儿术前、术后1周和3月的运动功能,并行统计学分析。结果术后CT复查显示:所有病儿毁损范围覆盖预定靶点。术后穿刺道少量出血1例,轻度发热1例,无严重手术并发症。术前、术后1周和术后3月GMFM-88评分分别为(49.9±23.5)、(52.6±24.0)、(63.8±22.4)分,与手术前比较,差异均具有统计学意义(P <0.01)。术后3个月,总有效率为98.2%,运动功能平均改善率为24.1%。结论采用机器人辅助无框架立体定向手术治疗儿童脑性瘫痪安全可靠,对改善病儿的运动功能具有积极意义。
Objective To investigate the safety and effectiveness of frameless stereotactic surgery in the treatment of children with cerebral palsy. Methods Clinical data of 109 children with spastic cerebral palsy were retrospectively analyzed. The patients were treated with robot-assistant frameless stereotactic surgery of nuclear mass destruction. The motor function was evaluated by gross motor function measure-88(GMFM-88) at the preoperation, and 1 week and 3 months after the operation respectively, then,statistical analysis was performed. Results The postoperative CT showed that the destruction area covered the predetermined target in all the patients. Minimal bleeding of puncture channel occurred in 1 patient and mild fever in 1, and there was no severe surgical complication occurred. The GMFM-88 scores were 49.9 ± 23.5 before the operation, and 52.6 ± 24.0 and 63.8 ± 22.4 at 1 week and3 months after the operation respectively, and there was a statistical difference between before and after the operation(P < 0.01). The total response rate was 98.2%, and the average improvement rate of motor function was 24.1% at 3 months after the operation.Conclusion The robot-assistant frameless stereotactic surgery is safe and reliable for spastic cerebral palsy in children, and has positive significance in improving patients’ motor function.
