胸腔积液中LDH、CEA、ADA联合检测对结核性和恶性胸腔积液的诊断价值 被引量:1

摘要 目的探讨胸腔积液中乳酸脱氢酶(LDH)、癌胚抗原(CEA)、腺苷脱氨酶(ADA)联合检测对结核性和恶性胸腔积液的诊断价值。方法回顾性分析中山市小榄人民医院2018年7月至2020年5月收治的30例胸腔积液患者的临床资料,其中结核性胸腔积液患者19例,设为结核性胸腔积液组,恶性胸腔积液患者11例,设为恶性胸腔积液组。对两组患者胸腔积液中LDH、CEA、ADA进行检测。将两组患者胸腔积液中LDH、CEA、ADA进行对比;分析单项及联合检测对结核性胸腔积液的诊断价值;分析单项及联合检测对恶性胸腔积液的诊断价值。结果恶性胸腔积液组患者LDH、CEA水平高于结核胸腔积液组,结核性胸腔积液组患者ADA水平高于恶性胸腔积液组;联合检测胸腔积液中LDH、CEA、ADA3项指标对结核性胸腔积液、恶性胸腔积液诊断的敏感性、特异性、阳性预测值、阴性预测值均高于各项指标单独检测(均P<0.05)。结论联合检测胸腔积液中LDH、CEA、ADA水平有助于鉴别结核性胸腔积液和恶性胸腔积液。 Objective To investigate the combined detection of lactate dehydrogenase(LDH),carcinoembryonic antigen(CEA)and adenosine deaminase(ADA)in pleural effusions for tuberculous and malignant pleural effusions the diagnostic value.Methods A retrospective analysis of the clinical data of 30 patients with pleural effusion in Xiaolan People’s Hospital of Zhongshan from July 2018 to May 2020,19 patients with pleural effusion were set as tuberculous pleural effusion group,and 11 patients with malignant pleural effusion were set as malignant pleural effusion group.The LDH,CEA,and ADA in the pleural effusion of the two groups of patients were detected.Compared the LDH,CEA and ADA in the pleural effusion of the two groups of patients;analyzed the diagnostic value of single item and combined detection for tuberculous pleural effusion;analysis sheet The diagnostic value of items and combined detection for malignant pleural effusion.Results The LDH and CEA levels of patients in the malignant pleural effusion group were higher than those in the tuberculosis pleural effusion group,and those in the tuberculous pleural effusion group,the level of ADA was higher than that of the malignant pleural effusion group;the combined detection of LDH,CEA and ADA in the pleural effusion is sensitive and specific to the diagnosis of tuberculous pleural effusion and malignant pleural effusion,heterosexuality,positive predictive value,and negative predictive value were all higher than those tested individually(all P<0.05).Conclusion The combined detection of LDH,CEA and ADA levels in pleural effusion is helpful for identification do not tuberculous pleural effusion and malignant pleural effusion.
