高血压脑出血患者血清HMGB-1、SF、ET-1水平变化及临床意义 被引量:2

Changes and Clinical Significance of Serum HMGB-1, SF, ET-1 Levels in Patients with Hypertensive Intracerebral Hemorrhage

摘要 目的:分析高迁移率族蛋白-1(HMGB-1)、铁蛋白(SF)、内皮素-1(ET-1)应用于高血压脑出血患者检测中的临床意义。方法:选取2018年1月-2020年8月本院共收治的高血压脑出血患者60例纳入观察组,根据NIHSS量表评分分为A组(NIHSS评分1~4分,轻度神经缺损患者)、B组(NIHSS评分5~20分,中度神经缺损患者)、C组(NIHSS评分>20分,重度神经缺损患者),各20例。再根据预后结果分为预后良好组(39例,NIHSS评分下降45%及以上)、预后较差组(21例,NIHSS评分下降45%以下或死亡)。另抽选同时段入院体检的健康人群23例纳入对照组。各组研究对象均接受血清HMGB-1、SF、ET-1水平检测,评价及对比各组间的HMGB-1、SF、ET-1水平。结果:观察组HMGB-1、SF、ET-1水平均高于对照组(P<0.05);A、B、C组HMGB-1、SF、ET-1水平均高于对照组(P<0.05),且三组间比较差异有统计学意义(P<0.05)。预后良好组HMGB-1、SF、ET-1水平均低于预后较差组(P<0.05)。结论:HMGB-1、SF、ET-1水平可准确反映高血压脑出血病情严重程度,且其指标水平均高于健康人群。HMGB-1、SF、ET-1水平越低,提示高血压脑出血预后良好。 Objective:To analyze the clinical significance of high mobility group protein-1 (HMGB-1),ferritin (SF) and endothelin-1 (ET-1) in the detection of hypertensive intracerebral hemorrhage patients.Method:A total of 60 patients with hypertensive intracerebral hemorrhage admitted to our hospital from January 2018 to August 2020 were selected and included in the observation group.According to the score of NIHSS scale,they were divided into group A (NIHSS score 1-4 points,patients with mild nerve defect),group B (NIHSS score 5-20 points,patients with moderate nerve defect),group C (NIHSS score >20 points,patients with severe nerve defect),20 cases in each group.According to the prognosis,they were divided into the good prognosis group (39 cases,NIHSS score decreased 45% or more) and the poor prognosis group (21 cases,NIHSS score decreased by less than 45% or death).Another 23 healthy people who were admitted to hospital for physical examination at the same period were selected and included into the control group.The subjects of each group received serum HMGB-1,SF,ET-1 levels to evaluate and compare the levels of HMGB-1,SF,ET-1 among the groups.Result:The levels of HMGB-1,SF and ET-1 in the observation group were higher than those in the control group (P<0.05);the levels of HMGB-1,SF and ET-1 in the groups A,B and C were higher than those in the control group (P<0.05),the differences among the three groups were statistically significant (P<0.05).The levels of HMGB-1,SF and ET-1 in the good prognosis group were lower than those in the poor prognosis group (P<0.05).Conclusion:The levels of HMGB-1,SF and ET-1 can accurately reflect the severity of hypertensive intracerebral hemorrhage,and their index levels are higher than those of healthy people.The lower the level of HMGB-1,SF,ET-1,it indicates that the prognosis of hypertensive intracerebral hemorrhage is good.
