摘要 目的研究基于PET/CT的分子影像技术在淋巴瘤个体化治疗中的应用。方法选取2017年6月至2018年6月我院收治的淋巴瘤患者73例为研究对象,接受个体化治疗,治疗前、治疗6个疗程后进行PET/CT和单独CT检查,测量标准摄取值(SUV),分析比较不同检查结果,评估对治疗效果的评定价值,并随访2年,分析SUV与近期无进展生存率(PFS)、总生存率(OS)的关系。结果 73例淋巴瘤患者,59例经临床随访证实,14例经手术病理证实,共240个病灶;PET/CT检查1处假阳性、1处假阴性,CT检查假阳性12处、假阴性36处。73例淋巴瘤患者经6个疗程治疗后,PET/CT阳性53例、阴性20例,CT检查17例假阳性、9例假阴性,准确率分别为62.29%、45.00%。三种类型淋巴瘤(弥漫大B细胞型、NK/T细胞型、粘膜相关淋巴组织型)患者治疗前后SUVmax值比较有显著差异(P<0.05);ΔSUVmax>78.9%对预测患者生存率的特异性和准确率较高(PFS:86.74%、81.06%;OS:84.52%,80.96%)。结论基于PET/CT的分子影像技术对淋巴瘤诊断价值优良,能有效评估患者治疗效果及近期生存率。 Objective To investigate the application of PET/CT-based molecular imaging technology in the individualized treatment of lymphoma.Methods 73 lymphoma patients were undergoing individualized treatment in our hospital from June 2017 to June 2018 were selected.PET/CT and CT exa minations were performed before and after 6 courses of treatment.The standa rd uptake value(SUV)was measured and compared.The value of SUV for treatment effect was analyzed.The relationship between SUV and recent progression-free su rvival(PFS)and overall su rvival(OS)was analyzed du ring the 2-year follow-up.Results Among the 73 patients with lymphoma,59 cases were confirmed by clinical follow-u p,and 14 cases we re confirmed by surgical pathology,with a total of 240 lesions.The re were 1 false positive case and 1 false negative case in PET/CT,12-false positive cases,and 36 falsenegative cases in CT.After 6 courses of treatment,PET/CT showed 53 positive cases and 20 negative cases.CT showed 17 false positive cases and 9 false-negative cases;The accu racy rate of PET/CT and CT was 62.29%and 45.00%,respectively.SUVmax values of the three types of lymphoma(diffuse large B-cell lymphoma,NK/T cell lymphoma,and mucosa-associated lymphoid tissue lymphoma)had significant differences before and after treatment(P<0.05).ΔSUVmax>78.9%has high specificity and accuracy in predicting the su rvival rate of patients(PFS:86.74%,81.06%;OS:84.52%,80.96%).Condusion PET/CTbased molecula r imaging technology has excellent diagnostic value for lymphoma,and ca n effectively evaluate the treatment effect and short-term survival rate of patients.
