脑出血患者血清可溶性白细胞介素6受体动态变化及与脑损伤程度的关系 被引量:1

摘要 目的探讨脑出血患者血清可溶性白细胞介素受体6(sIL-6R)动态变化及与脑损伤程度的关系。方法我院收治的90例脑出血患者为脑出血组,另选同期30例健康体检者为正常组,比较不同时间点两组及不同病情程度、出血量的脑出血患者的sIL-6R水平,分析血清sIL-6R水平对轻、重度脑损伤患者的临床鉴别价值。结果脑出血组sIL-6R水平高于正常组(P<0.05);脑出血组第3、5、7天sIL-6R水平低于脑出血12 h(P<0.05);重度组sIL-6R水平高于中轻度组;出血大量组sIL-6R水平高于中量组、小量组(P<0.05);脑出血3天时血清sIL-6R水平鉴别中轻、重度脑损伤的曲线下面积(AUC)为0.789,界值为888.13 ng/ml,敏感度与特异度为64.29%、80.00%。结论不同病情、出血量的脑出血患者血清sIL-6R水平在脑出血12 h后最高,此后呈动态变化,维持较高水平,且脑出血第3天血清sIL-6R水平可有效鉴别中轻、重度脑损伤,临床上可丰富脑损伤的监测手段。 Objective To investigate the dynamic changes of serum soluble interleukin-6 receptor(sIL-6R)in patients with cerebral hemorrhage and its relationship with the severity of brain injury.Methods Ninety patients with cerebral hemorrhage who were admitted to outpatient and emergency departments of our hospital were enrolled as cerebral hemorrhage group,while another 30 healthy controls were enrolled as normal group.The level of sIL-6R at different time points between the two groups and among the patients with different conditions and blood loss was compared.The clinical differential value of serum sIL-6R for patients with mild and severe brain injury was analyzed.Results The level of sIL-6R in the cerebral hemorrhage group was higher than that in the normal group(P<0.05).In the cerebral hemorrhage group,sIL-6R level onthe 3^(rd),5^(th)and 7^(th)day after cerebral hemorrhage was lower than that at 12 h after cerebral hemorrhage(P<0.05).The sIL-6R level in the severe group was higher than that in the mild group and the sIL-6R level in the heavy bleeding group was higher than that in the middle and small bleeding group(P<0.05).The area under the ROC curve(AUC),cutoff value,sensitivity and specificity of serum sIL-6R level onthe 3^(rd)day after cerebral hemorrhage for distinguishing moderate-mild from severe brain injury were 0.789,888.13 ng/ml,64.29%and 80.00%,respectively.Conclusion The level of serum sIL-6R is the highest at 12 h after cerebral hemorrhage in cerebral hemorrhage patients with different conditions and blood loss.Thereafter,it shows dynamic changes and maintains a high level.The level of serum sIL-6R onthe 3^(rd)day after cerebral hemorrhage can effectively distinguish moderate-mild from severe brain injury.It can enrich the monitoring methods of brain injury clinically.
