摘要
目的研究不同挂线法应用于临床复杂性肛瘘患者的疗效观察。方法选择我院2018年3月至2020年3月期间收入的70例复杂性肛瘘患者进行研究,按照挂线方式不同分为两组均35例,其中对照组采取传统实挂线法进行手术,研究组实施保留括约肌挂线法治疗,观察两组患者手术情况、肛门功能、疼痛评分、生活质量。结果术前两组患者肛门功能无差异,术前两组患者疼痛评分无差异,手术前两组患者生活质量评分无差异,P>0.05,术后2周、术后1个月肛门功能评分低于对照组,研究组手术时间、创面愈合时间、创伤面积、住院时间短于对照组,术后2周、术后1个月疼痛评分低于对照组,术后2周研究组生活质量评分高于对照组,差异显著,P<0.05。结论对于复杂性肛瘘患者可实施保留括约肌挂线法治疗,可降低患者疼痛,并缩短患者愈合时间,提升肛门功能评分,改善患者整体生活质量,值得应用。
Objective To study the effectiveness of different thread-ligating therapies in complex cryptoglandular anal fistulas.Methods 70 patients with complex cryptoglandular anal fistulas treated from March 2018 to March 2020 in our hospital were investigated and grouped by different thread-ligating therapies,35 cases in each group.The control group adopted the conventional thread-ligating method;the study group adopted the thread-ligating method with preserve of the sphincter.Operation conditions,anal function,pain scores and life quality were observed.Results Before surgery,the anal function,pain scores and life quality scores between groups were not significantly different (P>0.05).After 2w and 1m of surgery,anal function scores in the study group were lower than those in the control group.Compared with control group,the operation time,wound healing time,wound area,hospital stays as well as pain scores after 2w and 1m of surgery in the study group were obviously reduced;after 2w of surgery,life quality scores in the study group were significantly higher than those in the control group (P<0.05).Conclusion The thread-ligating method with preserve of the sphincter can relieve the pain,shorten the wound healing time,and improve the anal function scores and life quality.It is worthy of application.
