摘要
目的:探索轻、中度牙列拥挤患者采用无托槽隐形矫治器矫治过程中对牙周状态和龈沟液中炎性因子的表达情况。方法:收集2016年5月-2017年9月于笔者科室行口腔正畸伴有轻、中度牙列拥挤的40例患者为研究对象,将其按照随机数字表法分为实验组和对照组,每组20例。实验组予以无托槽隐形矫治法正畸,对照组则予以传统托槽矫治法正畸。分别在治疗前、治疗24个月时评估患者牙周状态,检测龈沟液中IL-1β、TNF-α、PGE2的表达水平。结果:治疗前,两组患者牙周指数及龈沟液中炎性因子的表达情况无明显差异(P>0.05);但治疗24个月后研究组患者牙周状态GI、SBI、PLI、CAL指数得分显著低于对照组(P<0.05),且治疗24个月后研究组中IL-1β、TNF-α及PGE2水平显著低于对照组(P<0.05)。结论:对于轻中度牙列拥挤患者,无托槽隐形矫治器较传统矫治器对牙周状态的影响更小,造成牙周炎症的程度更轻微,在临床有一定使用价值。
Objective To explore the expression of inflammatory factors in periodontal condition and gingival crevicular fluid during the treatment of patients with mild to moderate dentition with bracketless invisible appliance.Methods A total of 40 patients with orthodontic orthodontics in our department from May 2016 to September 2017 were collected.They were divided into experimental group and control group according to the random number table method,with 20 in each group.The experimental group was orthodontic without brackets,while the control group was orthodontic with traditional brackets.Before treatment,the periodontal status of the patients was evaluated at 24 months,and the expression levels of IL-1β,TNF-α,and PGE2 in the gingival crevicular fluid were detected.Results There were no significant differences in periodontal index and inflammatory factor expression in gingival crevicular fluid between the two groups of patients before treatment,(P>0.05).However,the GI,SBI,PLI,and CAL index scores of periodontal status in the study group were significantly lower than those in the control group after 24 months of treatment(P<0.05).The levels of IL-1β,TNF-αand PGE2 in the study group were significantly lower than those in the control group after 24 months of treatment(P<0.05).Conclusion For patients with mild to moderate dentition constriction,compared with traditional appliances,the bracket-less invisible appliance has a smaller impact on the periodontal state and causes a lesser degree of periodontal inflammation,which has certain clinical value.
