摘要
目的观察妊娠期糖尿病(GDM)"一日病房"综合管理对妊娠结局的影响.方法选择2018年1月至2019年12月在本院建册住院分娩的GDM孕妇120例,随机分为对照组行传统管理模式,观察组行"一日病房"综合管理,比较两组的孕期并发症及母婴结局.结果两组分娩方式无明显差异,观察组的孕期并发症、妊娠期高血压病、早产发生率明显低于对照组(P<0.05).母婴结局,两组胎膜早破、羊水异常、产后出血、胎儿窘迫发生率无显著差异,观察组巨大儿、新生儿低血糖的发生率较低(P<0.05).结论GDM"一日病房"综合管理可降低妊娠期高血压病、早产发生和巨大儿、新生儿低血糖的发生率,能改善妊娠期糖尿病母婴结局,值得推广.
Objective To assess the effect of comprehensive management with"one-day ward"during pregn.incy on the pregnancy outcome in patients with gestational diabetes.Methods A total of 120 pregnant patients with gestational diabetes,registered for delivery in our hospital from Jan 2018 to Dec 2019,were randomized into two groups;the control group was traditionally treated,and the observation group was given the"one-day ward"comprehensive management during pregnancy.An observation and comparison was performed for their complications and maternal and child outcomes.Results There was no significant difference in the delivery methods between the two groups.The incidences of pregnancy complications,hypertension and premature delivery,were significantly lower in the observation than in the control(P<0.05);although no significant difference was found for the incidences of premature mpture of membranes,the amniotic fluid,postpartum hemorrhage,and fetal distress.The incidences of hypoglycemia,giant infants were also lower in the observation(vs.control P<0.05).Conclusion The comprehensive management of GDM with"one-day ward"during pregnancy may reduce the incidences of hypertension during pregnancy,premature delivery,hypoglycemia in neonates,giant infants,and improve the outcome of maternal and neonatal gestational diabetes.It is worth recommended being as a clinical application.
