摘要
目的探究带状疱疹(HZ)患者辅助性T细胞17(Th17)/调节性T细胞(Treg)及相关细胞因子与疼痛程度及并发后遗神经痛(PHN)的关系。方法纳入本院皮肤科收治的168例HZ患者,其中44例合并PHN作为PHN组,其余124例患者作为HZ组,另选择同期健康志愿者80例作为对照组,流式细胞仪分析各组Th17、Treg细胞相对计数并计算Th17/Treg比率,酶联免疫吸附试验(ELISA)检测相关细胞因子及血清疼痛介质水平,Spearman检验分析各指标相关性。结果 PHN组VAS评分及病情程度评分显著高于HZ组,差异有统计学意义(P<0.05);3组WS、CS、HP及CP、SP、VGF和CGRP水平比较,PHN组>HZ组>对照组(P<0.05);β-EP、NT比较,PHN组>HZ组>对照组,差异均有统计学意义(P<0.05)。各组外周血Th17细胞、Th17/Treg及细胞因子IL-1β、IL-6、IL-17、IL-23、TNF-α水平比较,PHN>HZ>对照组,差异有统计学意义(P<0.05);各组外周血Treg细胞、IL-10、TGF-β1水平比较,PHN组<HZ组<对照组,差异具统计学意义(P<0.05)。h17/Treg、IL-1β、IL-6、IL-17、IL-10、IL-23、TNF-α、TGF-β1与PHN、VAS评分及神经损伤(WS、CS、HP、CP)存在相关性(P<0.05)。结论 HZ患者存在外周血Th17/Treg比率明显升高及相关细胞因子水平异常的情况,并与患者并发PHN、疼痛及病情加重和神经损伤密切相关。
Objective To explore the relationship between helper T cell 17(Th17)/regulatory T cell(Treg)and related cytokines,pain degree and postoperative neuralgia(PHN)in patients with herpes zoster(HZ).Methods Among the 168 patients with HZ admitted to the dermatology department of our hospital,44 patients were combined with PHN as the PHN group,the remaining 124 patients were regarded as the HZ group,and 80 healthy volunteers during the same period were selected as the control group.Flow cytometry was used to analyze the relative counts of Th17 and Treg cells in each group and the calculated Th17/Treg ratio,enzyme-linked immunosorbent assay(ELISA)to detect the levels of related cytokines and serum pain mediators,and Spearman test was used to analyze the correlation of each indicator.Results The VAS score and disease severity score of the PHN group were significantly higher than those of the HZ group,and the difference was statistically significant(P<0.05).Comparison of the levels of WS,CS,HP and CP,SP,VGF and CGRP in the three groups,PHN group>HZ group>control group(P<0.05).Comparison ofβ-EP and NT,PHN group>HZ group>control group,the difference was statistically significant(P<0.05).Comparison of the levels of peripheral blood Th17 cells,Th17/Treg and cytokines IL-1β,IL-6,IL-17,IL-23,TNF-αin each group,PHN>HZ>control group,the difference was statistically significant(P<0.05).The levels of Treg cells,IL-10 and TGF-β1 in peripheral blood of each group were compared,PHN group<HZ group<control group,the difference was statistically significant(P<0.05).Th17/Treg,IL-1β,IL-6,IL-17,IL-10,IL-23,TNF-α,TGF-β1 are related to PHN,VAS score and nerve damage(WS,CS,HP,CP)(P<0.05).Conclusion HZ patients have significantly higher peripheral blood Th17/Treg ratios and abnormal levels of related cytokines,which are closely related to patients with PHN,pain,aggravation,and nerve damage.
