摘要
目的系统评价循证护理对小儿手足口病合并脑炎患儿干预效果和安全性。方法检索文献数据库获取相关文献资料,中文数据库包括中国知网数据库(CNKI)、维普数据库(VIP)、万方数据库、中文生物医学文献数据库(CBM)等;外文数据库包括PubMed、EMBASE、NCBI、Medline、Web of Science、Cochrane Library等。检索时间为建库至2020年4月31日。搜索循证护理对手足口病合并脑炎患儿干预效果的研究文献,评价文献质量、提取数据并进行系统评价。结果系统评价显示循证护理对手足口病合并脑炎患儿干预效果显著,脑脊液检查白细胞水平明显降低[MD=-18.60,95%CI(-20.85,-16.35),P<0.001];蛋白含量明显下降[MD=-0.83,95%CI(-0.88,-0.77),P<0.001];葡萄糖水平明显降低[MD=-1.65,95%CI(-1.82,-1.48),P<0.001];氯化物水平无明显变化[MD=-0.50,95%CI(-5.06,4.07),P=0.83]。循证护理能显著减少手足口病合并脑炎患儿并发症发生率[RR=0.20,95%CI(0.12,0.34),P<0.001]。结论循证护理对手足口病合并脑炎患儿干预效果良好,可以显著减少腰穿不良反应、交叉感染和负面心理情绪的发生率,值得在临床护理实践中推广。
Objective To evaluate the effect and safety of evidence-based nursing in children with HFMD and encephalitis.Methods we searched literature database to get relevant literature.Chinese databases were CNKI,VIP,Wan fang,CBM,etc.foreign databases were PubMed,EMBASE,NCBI,MEDLINE,web of science,Cochrane Library,etc.The retrieval time is from the establishment of the database to April 31,2020.To search the literature about the intervention effect of evidence-based nursing on children with HFMD and encephalitis,evaluate the quality of literature,extract data and make a systematic evaluation.Results The system review showed that evidence-based nursing had a significant effect on the intervention of HFMD with encephalitis.The level of WBC in CSF was significantly reduced[MD=-18.60,95%CI(-20.85,-16.35),P<0.001],the content of protein was significantly reduced[MD=-0.83,95%CI(-0.88,-0.77),P<0.001],the level of glucose was significantly reduced[MD=-1.65,95%CI(-1.82,-1.48),P<0.001],there was no significant difference in chloride level[MD=-0.50,95%CI(-5.06,4.07),P=0.83].Evidence based nursing can significantly reduce the complication rate of HFMD with encephalitis[RR=0.20,95%CI(0.12,0.34),P<0.001].Conclusion Evidence-based nursing can significantly reduce the incidence of adverse reactions of lumbar puncture,cross infection and negative psychological emotions,and the patient’s satisfaction is good,which is worth promoting in clinical nursing practice.
