摘要
目的探究高强度聚焦超声(HIFU)消融治疗对子宫肌瘤患者性激素与临床疗效的影响。方法回顾性分析2017年1月至2019年2月广东东莞康华医院收治的有生育需求的96例子宫肌瘤患者临床资料,根据手术方式的不同分为研究组和常规组,每组各48例,研究组行HIFU子宫肌瘤消融术治疗,常规组接受腹腔镜子宫肌瘤剔除术治疗,比较两组患者术前1个月及手术3个月后的促卵泡激素(FSH)、促黄体生成激素(LH)、雌二醇(E2)三项性激素水平;比较两组治疗后12个月内的受孕情况与临床疗效。结果治疗前,两组FSH、LH、E2水平比较,差异无统计学意义(P>0.05),治疗后第3个月,研究组FSH、LH水平显著低于常规组,差异有统计学意义(P<0.05);治疗后12个月,研究组成功受孕率为45.83%,显著高于常规组的20.83%,差异有统计学意义(P<0.05);不良结局发生率为13.64%,显著低于常规组的60.00%,差异有统计学意义(P<0.05);两组临床治疗总有效率(89.58%vs.93.75%)比较,差异无统计学意义(P>0.05)。结论HIFU子宫肌瘤消融术治疗子宫肌瘤疗效与腹腔镜子宫肌瘤切除术相当,对患者性激素水平无显著影响,HIFU子宫肌瘤消融术有助于改善患者受孕情况和妊娠结局,值得临床推广。
Objective To investigate the impact of high intensity focused ultrasound(HIFU)ablation treatment on sex hormones and clinical efficacy in patients with uterine fibroids.Methods The clinical data of 96 patients with uterine fibroids admitted to our hospital from January 2017 to February 2019 were analyzed retrospectively.According to different surgical methods,they were divided into the study group(n=48)and the conventional group(n=48).Patients who underwent HIFU uterine fibroids ablation treatment were included in the study group,while patients who underwent laparoscopic myomectomy were included in the conventional group.The levels of follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)were compared between the two groups 1 month before operation and 3 months after operation.The pregnancy and clinical efficacy of the two groups within 12 months after treatment were compared.Results Before treatment,there was no statistically significant difference in FSH,LH and E2 levels between the two groups(P>0.05).While in the third month after treatment,FSH and LH levels in the study group were significantly lower than those in the conventional group,with statistically significant differences(P<0.05).After 12 months after treatment,the successful pregnancy rate in the study group was 45.83%,significantly higher than 20.83%in the conventional group,with statistically significant difference(P<0.05).The incidence of adverse outcomes was 13.64%,significantly lower than 60.00%of the conventional group,with statistically significant difference(P<0.05).The efficacy rates of clinical treatment between the two groups were(89.58%vs.93.75%),in which there was no statistically significant difference(P>0.05).Conclusion The therapeutic efficacy of HIFU uterine fibroids ablation treatment is equal to that of laparoscopic myomectomy,and it has no significant impact on the sex hormone level of patients.HIFU uterine fibroids ablation treatment is helpful to improve the pregnancy status and pregnancy outcome of patients,w
