摘要
先煎、久煎是毒性中药饮片煎煮的一大特色,然而考察毒性中药饮片先煎、久煎的研究现状时,发现2020年版《中国药典》记载的83种毒性中药饮片仅附子、制川乌、制草乌有先煎、久煎的要求,《医疗用毒性药品管理办法》记载的27种毒性中药饮片中,11种可入煎剂,但未涉及先煎、久煎的要求,相关文献报道了16种毒性中药饮片可以先煎、久煎。目前对需先煎、久煎的毒性中药饮片以及先煎、久煎的要求缺乏基本的认识,从而出现因毒性中药饮片煎煮不当导致的临床疗效不佳及不良反应等问题。本文结合2020年版《中国药典》、相关法规及文献,对先煎、久煎的毒性中药饮片品种、煎煮时间、毒性成分的变化等进行探讨,为煎药人员的实际操作提供依据,保证临床合理使用毒性中药饮片。
Pre-decoction and long time-decoction are major decoction features of toxic traditional Chinese medicine pieces.However,by investigating the research status of pre-decoction and long timedecoction only Aconiti Lateralis Radix Praeparata,Aconiti Radix Cocta and Aconiti Kusnezoffii Radix Cocta had requirements on pre-decoction and long time-decoction out of the 83 toxic traditional Chinese medicine pieces in the Chinese Pharmacopoeia(2020).Among the 27 toxic traditional Chinese medicine pieces recorded in the Measures for the Management of Toxic Drugs for Medical Use,11 was used as decoctions,but the requirements for pre-decoction and long time-decoction were not mentioned.Some literatures reported that 16 toxic traditional Chinese medicine pieces could be pre-decocted and long decocted.Basic understanding of toxic traditional Chinese medicine pieces that need pre-decoctand long decoction is lacking,as well as the requirements for the pre-decoction and long time-decoction.As a result,problems such as poor clinical efficacy and adverse reactions caused by improper decoction of toxic traditional Chinese medicine pieces arose.Based on the Chinese Pharmacopoeia(2020),relevant regulations and literatures,we discussed the varieties,length of decoction time and the changes of toxic components in traditional Chinese medicine requiring pre-decoction and long time-decoction,to help the decoction staff and ensure the rational use of toxic traditional Chinese medicine pieces.
