基于血清拉曼光谱技术对丙型肝炎病毒诊断和6a亚型的鉴别

摘要 目的建立血清拉曼光谱技术(RS)诊断丙型肝炎病毒(HCV)和6a亚型的鉴别方法。方法纳入165例HCV感染者为HCV组,120例同期健康体检者为非HCV组,采集受试者血清后用RS仪进行点测量,获得的数据用airPLS扣除荧光背景,使用PLS进行降维、简化处理,然后使用SVM对主成分数据进行判别分析,应用ROC曲线评估airPLS-PLS-SVM统计模型对测试集的诊断效果。结果HCV组与非HCV组300~3000 cm^(-1)位移范围内的血清RS图像形态相似,但部分位移处仍存在差异;RS诊断HCV的灵敏度为94.29%,特异度为96.15%,准确率为95.08%,曲线下面积(AUC)为0.953;其诊断6a亚型的灵敏度为100.00%,特异度为96.77%,准确率为97.18%,AUC为0.976。结论血清RS可用于HCV诊断和6a亚型的鉴别,有望成为HCV快速筛查及分型工具。 Objective To establish a method for distinguishing hepatitis C virus(HCV)from subtype 6a using serum Raman spectroscopy(RS).Methods A total of 165 cases of HCV were included in the HCV group,and 120 healthy people in the same period were involved in the non-HCV group.The serum of the subjects were collected and measured at different points using RS.The obtained data were subtracted from fluorescence background by airPLS.PLS was then used for dimension reduction and simplification,and then SVM was used for discriminant analysis of principal component data.The receiver operating characteristic curve was used to evaluate the diagnostic effect of airPLS-PLS-SVM statistical model on test set was evaluated by ROC curve.Results The HCV group and the non-HCV group had similar morphology of serum RS images in the shift range of 300-3000 cm^(-1),but there were still differences in some shifts.The sensitivity,specificity,accuracy and the area under curve(AUC)of RS in diagnosing HCV were 94.29%,96.15%,95.08%and 0.953,respectively.The sensitivity,specificity,accuracy and AUC of RS in diagnosing subtype 6a were 100.00%,96.77%,97.18%and 0.976,respectively.Conclusion Serum RS can be used for HCV diagnosis and the identification of subtype 6a,which is expected to be used as a tool for the rapid screening and typing of HCV.
