摘要
目的探讨不同材料充填乳前牙邻面龋对患儿口腔菌群及语言和咀嚼功能的影响。方法乳前牙邻面龋患儿100例,按随机数字表法分为观察组与对照组两组,每组各50例。对照组给予3M玻璃离子充填,观察组则给予含氟复合体充填,追踪两组充填前(T0)、充填即刻(T1)及充填一个月(T2)后充填体周围1mm范围内菌群结构改变情况以评估其微环境、语言和咀嚼功能的改善状态。结果T1、T2时两组充填体周围菌斑中的乳酸杆菌、放线菌及变形链球菌三种主要致龋菌的菌落数量均明显低于T0时刻(P<0.05);同时三种致龋菌各自占总菌落的百分率亦明显下降(P<0.05);但观察组三种主要致龋菌落的数量及占总菌落的百分率明显低于对照组(P<0.05)。两组患儿接受填充治疗后的咀嚼和语言功能的评分情况,观察组患儿的咀嚼和语言功能评分明显高于对照组,差异具有统计学意义(P<0.05),半年内观察组患牙发生松动脱落、牙龈炎、牙体变色情况低于对照组,两组比较差异具有统计学意义(P<0.01)。结论在修复乳前牙邻面龋的过程中,无论采用何种材料填充均可有效改善口腔菌群的微生态环境及语言和咀嚼功能,但复合体充填体在短期内能够显著抑制充填体龋洞周围菌斑中的主要致龋菌的比例,更易于提升修复效果。
Objective To investigate the effects of different backfill caries on oral flora structure,lauguage,chewing functions and immune microenvironment in children with anterior deciduous teeth.Methods A total of 100 children with anterior deciduous dental caries were divided into observation group and control group by random number table method,with 50 cases in each group.The control group was filled with 3 M glass ionomer,while the observation group was filled with fluorine-containing complex.The changes of bacterial flora structure within 1 mm around the backfill in the two groups were tracked before(T0),immediately(T1)and one month(T2)to evaluate the improvement of the microenvironment,language and chewing function.Results At T1 and T2,the number of colonies of lactobacillus,actinomycete and Streptococcus mutans,the three main caries causing bacteria,were significantly lower than that at T0(P<0.05).At the same time,the percentage of the three caries-causing bacteria in the total colony decreased significantly(P<0.05).However,the number and percentage of the three main caries causing colonies in the observation group were significantly lower than those in the control group(P<0.05).Two groups of children with filling treatment after ratings on chewing and language features,observation group with chewing and language function score significantly higher than the control group,difference has statistical significance(P<0.05),and a half years observation group have tooth mobility,gingivitis,tooth discoloration is lower than the control group,comparing the two groups statistically significant difference(P<0.01).Conclusion In the process of repairing the caries on the adjacent surface of the anterior deciduous teeth,no matter what materials are used for filling,the microecological environment,lauguage and chewing function of oral flora can be effectively improved,but the proportion of main caries causing bacteria in the plaque around the caries cavity of the backfill can be significantly inhibited in the short term,and the r
