摘要
目的了解河南省某脑血管病专科医院医疗服务半径及其影响因素,为医院未来发展明确战略定位,合理配置医疗资源提供参考依据方法采集该院2019年出院的21781份住院病案首页,对出院病人的年龄、性别、区域分布、疾病难度进行比较分析。采用相关分析法,探讨各地经济水平、人口老龄化程度、医疗技术水平、最短就诊路程与当地每万常住人口住院人数之间的关系结果该医院97.3%的患者来自省内,各地市转诊的患者病例难度系数高于医院所在地市每万常住人口住院人数与当地三级医院开放床位数之间呈负相关(r=-0.551,p=0.018),与人均可支配收入、老年系数、最短就诊路程之间不存在线性相关关系结论分级诊疗制度和当地的医疗技术水平是影响患者转诊到省级专科医院的主要因素省级专科医院要把握诊治疑难危重疾病的功能定位,提升医疗技术水平和学科能力,促进医院健康、协调、高质量发展。
Objective To understand the medical service range of a cerebrovascular disease hospital in Henan Province and its influencing factors,and provide reference for defining the strategic positioning of future development and reasonably allocating medical resources.Methods 21781 cases of inpatient medical records in 2019 were collected from the hospital.The age,gender,regional distribution,and disease difficulty of discharged patients were comparatively analyzed.The correlation analysis method was used to explore the relationship among the economic level,aging degree,medical technology,the shortest medical distance and the number of inpatients per 10000 resident populations.Results The difficulty coefficient of patients transferring from different cities was higher than that of the city where the hospital was located.There was a negative correlation between the number of sickbeds in local tertiary hospitals and the number of inpatients per 10000 resident populations(r=-0.551,p=0.018).There had no linear correlation among per capita disposable income,elderly coefficient,the shortest distance of treatment and the number of inpatients per 10000 resident populations.Conclusion The hierarchical diagnosis and treatment system and medical technology are the most important factors of referral to provincial hospitals.Provincial specialized hospitals should grasp the function of diagnosis and treatment of difficult and critical diseases,and improve medical technology to promote the hospitals'development.
