摘要
目的研究新式垂直悬吊皮肤牵引治疗患儿(≤2周岁)股骨干骨折的护理。方法回顾性分析2015年1月-2017年12月收住26例(≤2周岁)的股骨干骨折的患儿,均采用新式双下肢垂直悬吊皮牵引保守治疗。牵引前要做好患儿局部皮肤评估和清洁准备;加强家长健康教育,鼓励合理饮食,指导二便管理,预防患儿上呼吸道感染;牵引中注重患儿牵引观察患儿的反应和血液循环情况。牵引后单髋人字支具加强外固定的护理,并观察其疗效。结果26例患儿,全部患儿都接受了随访,最短6个月,最长至15个月,患儿的平均随访时间为(9.5±3.4)个月。经过4~8周的治疗后,骨折可恢复至骨性愈合,平均愈合时间(6.6±2.3)周。没有出现肢体的血运障碍,双下肢皮肤过敏、或起水泡等异常症状。共有7例患儿出现了患病肢体生长过度的情况,7例患儿的肢体过度生长程度各有不同,但所有过度生长的肢体长度都小于1.0 cm。结论新式垂直悬吊皮肤牵引用于治疗患儿(≤2周岁)股骨干骨折为非常有效的方法,而且治疗过程简单,并发症少,治疗后护理方便,效果明显。
Objective To explore nursing experience of infant(no more than 2 years old)femoral shaft fracture treated by new suspension skin traction.Methods A retrospective analysis of 26 infants(no more than 2 years old)with femoral shaft fractures from January 2015 to December 2017 were treated conservatively with a new type of vertical suspension skin traction for both lower limbs.Before traction,prepared for local skin assessment and cleansing of children,strengthened parent health education,encouraged reasonable diet,guided the management of the second stool,and prevented children from catching cold,paid attention to traction and observed the reaction and blood circulation of infants and young children during traction.After traction,the single hip herringbone brace strengthened the nursing of external fixation,and observed its effect.Results Of the 26 pediatric patients,all pediatric patients were followed up for the shortest 6 months and the longest 15 months.The average follow-up time for pediatric patients was(9.5±3.4)months.After 4 to 8 weeks of treatment,the fracture can be restored to bone union,the average time is(6.6±2.3)weeks.There were no abnormal symptoms such as blood flow disorder of the limbs,skin allergies of the lower limbs,or blisters.A total of 7 pediatric patients had overgrowth of the diseased limbs.7 pediatric patients had different degrees of overgrowth of their limbs,but all the overgrowth limbs were less than 1.0 cm in length.Conclusion The new vertical suspension skin traction for the treatment of femoral shaft fractures in infants and young children(no more than 2 years old)is a very effective method,and the treatment process is simple,the complications are few,and the post-treatment care is convenient,and the effect is obvious.
