摘要
目的探讨改良悬吊皮肤牵引结合小夹板治疗婴幼儿(≤3周岁)股骨干骨折的疗效。方法选取2016年1月至2018年8月本科收治的24例股骨干骨折患儿为观察对象,骨折闭合手法整复后采用医用自黏弹性绷带改良悬吊皮肤牵引结合小夹板治疗婴幼儿股骨干骨折。牵引2~3周,定期复查床边X线片,待骨折端骨痂生成明显后,去除牵引装置,患肢继续使用夹板固定2~3周。结果24例均获随访,平均随访时间6个月(3~9个月)。骨折全部达骨性愈合,愈合时间为4~8周。均无皮肤过敏和水泡发生,无末梢血液循环障碍。4例患儿患侧肢体有不同程度的过度生长,但长度<1.0 cm。患肢的力线正常,无明显旋转、成角畸形,髋膝关节活动度正常,无跛行。结论医用自黏弹性绷带改良悬吊皮肤牵引结合小夹板治疗婴幼儿(≤3周岁)股骨干骨折,简单有效、安全方便、并发症少。
Objective To investigate the effect of modified suspension skin traction combined with small splint in the treatment of femoral shaft fracture in≤3 years old infants.Methods 24 infants with femoral shaft fracture were treated with modified suspension skin traction with self-viscoelastic bandage and small splint fixation.After 2-3 weeks’traction,the infants were re-examined by bedside X-ray regularly.After the callus formation at the fracture end was obvious,the traction devices were removed and the affected limbs were fixed with splints for 2-3 weeks.Results All the 24 cases were followed up for 3-9 months,with an average of 6 months.Bone healing was achieved in all the cases;and the healing time was 4-8 weeks.There were no skin allergies,blisters,and peripheral blood circulation disorders.The limbs of 4 children had excessive growth in varying degrees,but the length was less than 1.0 cm.The force lines of the affected limbs were normal.There was no obvious deformity of rotation and angulation.The range of motion of the hip and knee joints was normal.There was no limp.Conclusion Modified suspension skin traction with medical self-viscoelastic bandage combined with small splint for femoral shaft fracture in≤3 years old infants is simple,effective,safe,and convenient,with fewer complications.
