摘要
目的:探讨改良口腔护理联合中药在ICU经口气管插管患者中的应用体会及价值。方法:将乌鲁木齐沙依巴克区市中医医院2017年10月-2019年10月重症监护室(Intensive Care Unit,ICU)经口气管插管患者100例,随机分组,改良口腔护理组患者实施改良口腔护理,中药联合改良口腔护理组对患者采取改良口腔护理联合中药护理。比较两组护理满意度;口腔异味消失时间、急性生理与慢性健康评分(Acute Physiology,Age and Chronic Health Evaluation,APACHE Ⅱ)评分;治疗前后菌落数目、牙菌斑指数、口腔异味指数;呼吸机相关性肺炎发生率。结果:中药联合改良口腔护理组满意度达到100%,高于改良口腔护理组76%(P<0.05)。治疗前改良口腔护理组、中药联合改良口腔护理组菌落数目、牙菌斑指数、口腔异味指数比较(P>0.05);治疗后中药联合改良口腔护理组菌落数目、牙菌斑指数、口腔异味指数均显著低于改良口腔护理组(P<0.05)。中药联合改良口腔护理组口腔异味消失时间、APACHEII评分低于改良口腔护理组相应的指标,P<0.05。中药联合改良口腔护理组呼吸机相关性肺炎发生率低于改良口腔护理组(P<0.05)。结论:改良口腔护理联合中药护理对于ICU经口气管插管效果确切,可改善口腔异味,去除牙菌斑和降低呼吸机相关性肺炎的发生率。
Objective:To explore the application experience and value of improved oral care combined with traditional Chinese medicine in ICU patients with oral tracheal intubation.Methods:A total of 100 ICU patients with oral tracheal intubation in Urumqi Shaybak District Hospital of Traditional Chinese Medicine from October 2017 to October 2019 were randomly divided into two groups.The patients in the modified oral care group were given improved oral care.The Chinese medicine combined with the modified oral care group was treated with modified oral care combined with traditional Chinese medicine.The nursing satisfaction of the two groups were compared;the oral odor disappearance time,APACHEⅡ score;the number of colonies,plaque index,oral odor index before and after treatment;the incidence of ventilator-associated pneumonia were compared.Results:The satisfaction rate of the Chinese medicine combined with the improved oral care group reached 100%,which was 76%higher than that of the modified oral care group(P<0.05).Comparison of the number of colonies,dental plaque index,and oral odor index of the modified oral care group and the Chinese medicine combined with improved oral care group before treatment(P>0.05);after treatment,the number of colonies,dental plaque index,and oral odor index of the combined oral care group of Chinese medicine and improved were significantly lower than those of the modified oral care group(P<0.05).The oral odor disappearance time and APACHEⅡ score of the traditional Chinese medicine combined modified oral care group were lower than the corresponding indexes of the modified oral care group(P<0.05).The incidence of ventilator-associated pneumonia in the Chinese medicine combined with improved oral care group was lower than that in the modified oral care group(P<0.05).Conclusion:Improved oral care combined with traditional Chinese medicine care is effective in ICU oral tracheal intubation,which can improve oral odor,remove dental plaque,and reduce the incidence of ventilator-associated pneumonia.
