Effect of Predictive Nursing Intervention Guided by Evidence-based Theory on Ventilator-associated Pneumonia in Patients with Mechanical Ventilation

摘要 目的:探讨以循证理论支持为导向的预见性护理干预对机械通气患者呼吸机相关性肺炎(VAP)发生情况的影响。方法:选取2018年8月-2020年8月本院ICU收治患者60例为研究对象,按随机数字表法将其分为观察组及对照组,各30例。所有患者均行机械通气,对照组给予常规护理干预,观察组给予以循证理论支持为导向的预见性护理干预。比较两组干预1周后呼吸机脱机率与VAP发生率。比较两组干预前及干预1周后血气指标[血氧饱和度(SpO_(2))、动脉血氧分压(PaO_(2))及二氧化碳分压(PaCO_(2))]与口腔清洁度[Beck口腔评分量表(BOAS)评分与牙菌斑指数]。结果:干预1周后,观察组呼吸机脱机率高于对照组,且VAP发生率低于对照组(P<0.05)。干预1周后,两组SpO_(2)与PaO_(2)均高于治疗前,且观察组均高于对照组(P<0.05)。干预1周后,两组PaCO_(2)均低于治疗前,且观察组低于对照组(P<0.05)。干预1周后,两组BOAS评分与牙菌斑指数均低于治疗前,且观察组均低于对照组(P<0.05)。结论:以循证理论支持为导向的预见性护理干预可显著降低机械通气患者VAP发生率、减少呼吸机依赖,显著提升血氧功能,改善口腔清洁度。 Objective:To explore the effect of predictive nursing intervention guided by evidence-based theory on the incidence of ventilator-associated pneumonia(VAP)in patients with mechanical ventilation.Method:A total of 60 patients admitted to ICU in our hospital from August 2018 to August 2020 were selected as study subjects.They were divided into observation group and control group according to random number table method,with 30 patients in each group.All patients were treated with mechanical ventilation.The control group was given routine nursing intervention,and the observation group was given predictive nursing intervention guided by evidence-based theory.The rate of ventilator removal and VAP after 1 week intervention were compared between the two groups.Blood gas indicators(SpO_(2),PaO_(2) and PaCO_(2))and oral cleanliness[Beck oral scale score(BOAS)and dental plaque index]were compared between the two groups before and 1 week after intervention.Result:After 1 week of intervention,the rate of ventilator removal in the observation group was higher than that in the control group,and the incidence of VAP was lower than that in the control group(P<0.05).After 1 week of intervention,SpO_(2) and PaO_(2) in both groups were higher than those before treatment,and those in the observation group were higher than those in the control group(P<0.05).After 1 week of intervention,PaCO_(2) in both groups were lower than those before treatment,and that in the observation group was lower than that in the control group(P<0.05).After 1 week of intervention,BOAS score and plaque index in both groups were lower than those before treatment,and the observation group were lower than those in the control group(P<0.05).Conclusion:Predictive nursing intervention guided by evidence-based theory can significantly reduce the incidence of VAP in patients with mechanical ventilation,reduce ventilator dependence,significantly improve blood oxygen function,and improve oral cleanliness.
