摘要
结节性痒疹是一种具有疣状结节性损害的慢性瘙痒性皮肤病,具有顽固难愈和剧烈瘙痒的特点,严重影响患者的正常生活。结节性痒疹的病因病机现代医学尚未阐明清楚,治疗虽快速有效但易于复发。而中医药安全有效、不良反应小、有效防止复发,治疗结节性痒疹具有优势和前景。现代医家多认为结节性痒疹的病机当责之于湿、瘀,笔者认为其与痰邪同样关系密切。该文从痰入手,分别从痰为病理产物和致病因素2个方面论述了痰邪与结节性痒疹的联系。认为从痰论治结节性痒疹的基本方为二陈汤加白芥子,同时提出了清热燥湿、健脾化痰和活血解毒、化痰软坚两种从痰论治结节性痒疹的思路及方法,并附医案1则,以冀有助于临床审因论治及选方用药,开阔从痰论治结节性痒疹的临床思路。
Prurigo nodosa is a chronic pruritic skin disease with verrucous nodular damage.It has the characteristics of stubbornness,hard to heal and severe itching,which seriously affects the normal life of patients.The etiology and pathogenesis of prurigo nodularis has not been clarified in modern medicine.Although the treatment is fast and effective,it is easy to relapse.Traditional Chinese medicine is safe and effective,has low side effects,and effectively prevents recurrence.It has advantages and prospects for treating prurigo nodularis.Most modern physicians believe that the pathogenesis of prurigo nodularis should be blamed on dampness and stasis.The author believes that it is also closely related to phlegm.This article starts with sputum and discusses the relationship between phlegm evil and prurigo nodularis from two aspects:sputum as a pathological product and pathogenic factors.It is believed that the basic prescription for treating prurigo nodularis from the phlegm theory is Erchen Decoction plus white mustard seeds.At the same time,two ideas for the treatment of prurigo nodularis from the phlegm theory are proposed for clearing heat and removing dampness,strengthening the spleen and resolving phlegm,promoting blood circulation and detoxification,and resolving phlegm and softening firmness,together with 1 medical record,in order to help clinical reason,treatment and prescription selection,broaden the clinical thinking of treating prurigo nodulare from phlegm theory.
