摘要
目的分析曲靖市妇幼保健院医疗不良事件的管理和报告现状,为完善妇幼医疗机构不良事件管理提出改进建议,进一步提升医院医疗质量与安全管理水平,确保患者享受高质安全的医疗服务。方法查阅曲靖市妇幼保健院内部资料,归纳总结不良事件报告现状,对医疗不良事件管理进行分析;收集曲靖市妇幼保健院2014年1月—2016年12月医疗不良事件监测数据,采取分层随机抽样调查开展医院职工问卷调查,对医疗不良事件发生构成和报告情况进行统计描述分析。结果医院2014—2016年医疗不良事件上报率(上报例数/出院人次)分别为1.47%,1.63%,1.48%;参与调查的人员有28.04%没有接受过不良事件相关培训,10%~18%的医院职工不知道院内不良事件上报制度和流程,主动上报过不良事件的员工仅占31.57%;医疗器械不良报告和药品不良反应报告占了全部医疗不良事件的61.62%。结论医院存在医疗不良事件报告系统不够完善、管理机制尚不健全、相关培训不足、应对新形势下医疗不良事件报告能力不足等问题。提出进一步完善医疗不良事件上报系统、完善报告制度、营造非惩罚性环境、构建培训体系、提升妇幼专科医疗不良事件管理水平的改进建议,促进减少医疗不良事件发生,维护患者就医安全。
Objective To analyze the current situation of management and reporting of adverse medical events in Qujing Maternal and Child Health Hospital,and to propose improvements to improve the management of adverse events in maternal and child medical institutions,to further improve the hospital's medical quality and safety management level,and to ensure that patients enjoy high-quality and safe medical services.Methods To consult the internal data of Qujing Maternal and Child Health Hospital,summarize the status of adverse event reports,and analyze the management of medical adverse events;collect the medical adverse event monitoring data of Qujing Maternal and Child Health Hospital from January 2014 to December 2016,and adopt stratified randomization sampling surveys and questionnaire surveys of hospital staff,and conduct statistical description analysis on the composition and reporting of adverse medical events.Results The hospital’s reporting rate of malpractice time(number of reported cases/discharged)from 2014 to 2016 were 1.47%,1.63%,and 1.48%,respectively;28.04%of the surveyed personnel did not receive training related to adverse events,10%-18%of hospital staff do not know the system and process for reporting adverse events in the hospital,and only 31.57%of employees have actively reported adverse events;adverse medical device reports and adverse drug reaction reports accounted for 61.62%of all medical adverse events.Conclusion The hospital has problems such as insufficient medical adverse event reporting system,insufficient management mechanism,insufficient relevant training,and insufficient ability to respond to medical adverse event reporting under the new situation.To further improve the medical adverse event reporting system,improve the reporting system,create a non-punitive environment,build a training system,and improve the management level of adverse medical events in women and children specialties,promote the reduction of adverse medical events,and maintain the safety of patients are proposed.
