摘要
目的通过统计儿童社区获得性肺炎住院治疗的抗菌药物,评价使用不同治疗方案的药物经济学,为儿童合理、经济用药提供参考。方法收集中国医科大学附属盛京医院2017年1月1日至2018年12月31日,诊断为“社区获得性肺炎”且住院治疗的患儿的资料。根据患儿用药方案的差异,将小儿肺炎患者分为联合治疗和单药治疗。运用药物经济学的最小成本分析法,分别对各方案的经济性进行评价。结果联合治疗方案中,红霉素联合盐酸头孢甲肟方案的总治疗费用最高(7654.96元),阿奇霉素联合头孢美唑钠方案的总治疗费用最低(5315.02元);单药治疗方案中,红霉素方案的总治疗费用最高(7467.09元),头孢呋辛钠方案的总治疗费用最低(4672.32元)。所有治疗方案中,红霉素联合盐酸头孢甲肟方案的总治疗费用最高,头孢呋辛钠方案的总治疗费用最低,两个方案之间差异为2982.64元。各治疗方案比较,住院天数差异有统计学意义(P<0.01)。所有治疗方案中,阿奇霉素联合头孢他啶方案患儿的住院天数最短,为5.6 d,其余方案的住院天数以6~8 d内居多。结论对于儿童社区获得性肺炎患儿,单独采用头孢呋辛钠治疗,在获得满意的疗效的同时,可获得有最佳的经济效果。
Objective To evaluate the pharmacoeconomic effects of different drug regimens by statistical analy sis of antibiotics used in hospitalized children with community-acquired pneumonia(CAP),so as to provide reference for rational and economical drug use in children.Methods The medication information of children diagnosed with community-acquired pneumonia and hospitalized from January 1,2017 to December 31,2018 in Shengjing Hospital of China Medical University was retrieved.According to the difference in medication schemes for children,335 cases of children with pneumonia were divided into combination treatment and single drug treatment.The cost-minimization analysis method of pharmacoeconomics was used to evaluate the economy of each scheme.Results In the schemes of combination treatment,total treatment cost of erythromycin combined with cefmetaxime hydrochloride was the highest(¥7654.96),and that of azithromycin combined with cefmetazole sodium was the lowest(¥5315.02);in the schemes of single drug treatment,the total treatment cost of erythromycin was the highest(¥7467.09)and that of cefuroxime sodium was the lowest(¥4672.32).Among all the treatment schemes,the total cost of erythromycin combined with cefmexime hydrochloride was the highest,while that of cefuroxime sodium was the lowest;the difference between the two treatments was¥2982.64.There was a significant difference in the number of hospitalization days among different treatment schemes(P<0.01).Among all the treatments,the number of hospitalization days of azithromycin combined with ceftazidime was the lowest,which was 5.6 days,and most of the rest was within 6~8 days.Conclusion For children with community-acquired pneumonia,cefuroxime sodium alone can obtain satisfactory curative effect and the best economic effect.
