摘要
目的探讨FGA、HSP90α、SPP1联合对HBV相关性肝细胞癌的早期诊断价值。方法纳入早期HBV相关性肝细胞癌患者70例为肿瘤组、肝硬化患者70例为肝硬化组、健康体检者70例为对照组;检测肿瘤组、肝硬化组和对照组血清中FGA、HSP90α、SPP1的表达量;用受试者工作特征曲线(receiver operating characteristic curve,ROC)的AUC分析FGA、HSP90α、SPP1检测在HBV相关性肝细胞癌中的诊断意义。结果HBV相关性肝细胞癌患者血清中FGA、HSP90α、SPP1的表达量均显著高于肝硬化患者(P<0.05)和健康体检者(P<0.05);肝硬化患者血清中FGA的表达量高于健康体检者(P<0.05)。区分早期HBV相关性肝细胞癌患者与健康体检者的FGA、HSP90α、SPP1的灵敏度分别为87%,80%,83%,特异度分别为83%,87%,83%。血清中FGA、HSP90α、SPP1联合对HBV相关性肝细胞癌的诊断的的灵敏度为88%,特异度为90%,准确度为89%。结论血清中FGA、HSP90α、SPP1的表达量可作为诊断早期HBV相关性肝细胞癌的潜在标志。
Objective To investigate the early diagnostic value of FGA,HSP90αand SPP1 in the diagnosis of HBV-associated hepatocellular carcinoma.Methods 70 patients with early HBV-related hepatocellular carcinoma were as tumor group,70 patients with cirrhosis were as cirrhosis group,and 70 healthy people were as the control group.The serum and FGA,HSP90αwere detected in the tumor group,cirrhosis group and the control group.The expression level of SPP1;the AUC of the receiver operating characteristic curve(ROC)was used to analyze the diagnostic significance of FGA、HSP90α、SPP1 in HBV-associated hepatocellular carcinoma.Results The expression levels of FGA,HSP90αand SPP1 in serum of HBV-associated hepatocellular carcinoma patients were significantly higher than those in patients with liver cirrhosis(P<0.05)and healthy subjects(P<0.05).The expression of FGA in serum of patients with liver cirrhosis was higher than that of healthy subjects(P<0.05).The sensitivity of FGA,HSP90α,and SPP1 in patients with early HBV-associated hepatocellular carcinoma and healthy subjects were 87%,80%,and 83%,respectively,and the specificities were 83%,87%,and 83%,respectively.The sensitivity of serum FGA,HSP90α,and SPP1 in the diagnosis of HBV-associated hepatocellular carcinoma was 88%,specificity was 90%,and accuracy was 89%.Conclusion The detection of FGA,HSP90αand SPP1 in serum can be used as a potential marker for the diagnosis of early HBV-associated hepatocellular carcinoma.
