摘要
目的观察短杠杆手法配合牵引对腰椎间盘突出症患者疼痛、腰椎间孔横径的影响。方法将本院收治的90例椎间盘突出症患者随机分为对照组与实验组各45例,对照组采用常规推拿肌肉松解疗法,实验组在对照组治疗基础上加用短杠杆手法配合牵引进行治疗。比较两组患者治疗前后腰椎影像学参数、腰痛视觉模拟评分(VAS)和Oswestry功能障碍指数(ODI)评分,临床疗效与生活质量。结果实验组临床有效率显著高于对照组(P<0.05);治疗后,两组VAS、ODI评分均低于治疗前且实验组VAS、ODI评分均低于对照组(P<0.05);实验组腰椎间孔前后径、侧隐窝宽度均高于对照组,腰椎间孔横径低于对照组(P<0.05);实验组生活质量量表(SF-36)评分均高于对照组(P<0.05)。结论短杠杆手法配合牵引治疗腰椎间盘突出症患者可有效加快患者腰椎功能恢复,降低VAS和ODI评分,改善腰椎间孔横径和前后径,提高生活质量。
Objective To observe the effects of short lever manipulation combined with traction on pain and lumbar foraminal transverse diameter in patients with lumbar disc herniation(LDH).Methods A total of 90 LDH patients were randomly divided into two groups 45 cases in each group.Control group underwent routine massage muscle relaxation therapy,while experimental group was additionally treated with short lever manipulation combined with traction.The lumbar imaging parameters,scores of visual analogue scale(VAS) and Oswestry dysfunction index(ODI),clinical effect and quality of life before and after treatment were compared between the two groups.Results The effective rate of clinical treatment in experimental group was higher than that in control group(P<0.05);after treatment,scores of VAS and ODI in both groups were decreased,which were lower in experimental group than control group(P<0.05);lumbar foraminal anterior-posterior diameter and lateral recess width in experimental group were higher than those in control group,while lumbar foraminal was lower than that in control group(P<0.05);scores of short form 36 health survey(SF-36) in experimental group were higher than those in control group(P<0.05).Conclusion Short lever manipulation combined with traction in the treatment of LDH patients can effectively accelerate the recovery of lumbar function,reduce scores of VAS and ODI,improve lumbar foraminal transverse diameter and anterior-posterior diameter,and improve the quality of life.
