摘要
目的总结软骨肉瘤(chondrosarcoma)的影像学异常征象和影像学特征,提高软骨肉瘤的诊断水平和认识程度。方法回顾性分析经临床手术病理证实的38例软骨肉瘤的影像学表现。所有患者均行X线平片检查,其中行CT检查者20例,行MRI检查者20例,行NM检查者10例。结果 X线平片显示病灶内有钙化者30例,CT检查者均可见不同程度的骨质破坏和大小不一的钙化影,其中伴有软组织肿块者25例,可见骨膜反应者3例。20例行MRI检查者显示髓腔受累范围均比CT显示的范围大且清晰,肿瘤实质在T1WI上呈等、低信号,T2WI上根据恶性高低而分别表现为不均匀或均匀高信号,增强后大部分病灶呈不均匀、分割样强化。结论不同分型的软骨肉瘤的影像学表现具有不同影像学特征,同时也具有一定共性,结合不同影像学检查结果可提高本病的诊断水平。
Objective To summarize the abnormal imaging signs and imaging features of chondrosarcoma so as to improve the diagnostic level and understanding extent of chondrosarcoma.Methods The imaging findings of 38 cases of chondrosarcoma confirmed by clinical operation and pathology were retrospectively analyzed.All patients received plain X-ray examination,among which,CT examination was performed in 20 cases,MRI examination in 20 cases,and NM examination in 10 cases.Results X-ray plain film showed calcification in the lesion in 30 cases.CT examinees showed different degrees of bone destruction and calcification of different sizes,among which there were 25 cases with soft tissue mass and 3 cases with periosteum reaction.All the 20 patients underwent MRI examination showed a larger and clearer range of medullary cavity involvement than that shown by CT.The tumor parenchyma showed isointense and hypointense on T1WI,while the tumor parenchyma showed heterogeneous or homogeneous hyperintense on T2WI according to the level of malignancy.Most of the lesions showed heterogeneous and segmented enforcement after enhancement.Conclusion The imaging manifestations of different types of chondrosarcoma have different imaging characteristics,and also have certain commonalities.Combining different imaging findings can improve the diagnostic level of this disease.
