脂联素基因多态性与子宫内膜异位症的相关性 被引量:7

摘要 目的:探讨脂联素(ADIPOQ)基因的多态性是否与中国河南地区汉族育龄期妇女子宫内膜异位症(EMs)相关。方法:采用病例-对照研究设计,收集282例EMs患者和271例无内异症妇女的外周血,采用基质辅助激光解析电离飞行时间质谱(MALDI-TOFMS)法对脂联素基因rs822395、rs17366568、rs2241766和rs1501299四个SNP位点进行基因分型。结果:经年龄和BMI校正后,rs2241766位点携带GG基因型致EMs效应是TT基因型的0.71倍(P=0.040,95%CI=0.51~0.98),加性模型和显性模型下rs2241766位点的携带G等位基因均与EMs发生风险降低有关(OR分别为0.71、0.68,均P<0.05);rs1501299等位基因T与对照组差异有统计学意义(加性模型P=0.050,OR=0.75,95%CI=0.57~1.00)。rs2241766位点的GT+GG基因型与EMs患者临床分期、CA125水平、有无痛经相关(OR分别为0.026、0.40、0.55,P<0.05)。rs1501299基因上携带T等位基因可使EMs患者高CA125水平、不孕发生的风险降低(OR分别为0.49、0.60,P<0.05)。结论:脂联素基因rs2241766和rs1501299多态性与河南地区育龄期妇女子宫内膜异位症易感性相关。 Objective:To evaluate whether adiponectin(ADIPOQ)gene single nucleotide polymorphisms(SNPs)were associated with endometriosis(EMs)risk in Henan Han cohort.Methods:We performed a case-control study including 282 EMs patients and 271 controls with peripheral blood.SNPs locis(rs822395、rs17366568、rs2241766 and rs1501299)in ADIPOQ were determined using a Sequenom MassArray system.Association analysis was calculated using the unconditional logistic regression model.Result:After adjustments for age and BMI,the GG genotype of rs2241766 reduced EMs risk by 0.71-fold confers a lower risk of EMs than TT genotype(P=0.040,OR=0.71,95%CI=0.51~0.98),while G allele confers a lower risk of EMs in the additive and dominant model(OR=0.71、0.68,P<0.05).Similarly,rs1501299 T was associated with a reduced risk of EMs in the additive models(P=0.050,OR=0.75,95%CI=0.57~1.00).EMs patients individuals carrying the rs2241766 GG+GT genotype reduced the risk of a high CA125 level、dysmenorrhea and advanced stage(OR=0.026、0.40、0.55,P<0.05),and T allele at rs1501299 as a protective factor that associated with reduced frequency of high CA125 level and infertility(OR=0.49、0.60,P<0.05)and in EMs patients.Conclusion:ADIPOQ gene rs2241766 and rs1501299 was associated with endometriosis susceptibility.
