摘要
近年来,随着临床医学技术的不断提升与发展,超声引导下臂丛置管神经阻滞在上肢手术术后镇痛中逐渐得到应用和发展。臂丛置管神经阻滞是一种把局麻药物注射到C5~8和T1脊神经的前支组成并支配上肢的感觉和运动区域,使其通过神经传导产生麻醉作用的一种方法。相比于神经刺激定位法和神经穿刺异感法,超声引导臂丛置管神经阻滞置管更精准、创伤更小、术后镇痛效果更佳,并且与传统的静脉自控镇痛相比,超声引导臂丛置管持续镇痛的效果也更理想,且药物不良反应较少,患者满意度更高。鉴于此,该文现就超声技术基础知识、超声引导臂丛置管方式以及臂丛置管术后常用镇痛药物3个方面对超声引导臂丛置管神经阻滞在上肢手术术后镇痛的研究进展进行概述。
In recent years,with the continuous improvement and development of clinical medical technology,ultrasoundguided lower brachial plexus catheterization has gradually been applied and developed in postoperative analgesia of upper limb surgery.Brachial plexus catheterization nerve block is a method of injecting local anesthetics into the anterior branches of the C5-8 and T1 spinal nerves and innervating the sensory and motor areas of the upper limbs to produce anesthesia through nerve conduction.Compared with the nerve stimulation positioning method and the nerve puncture parasitic method,the ultrasound-guided brachial plexus catheterization nerve block catheterization is more accurate,less traumatic,and postoperative analgesia was better.Compared with traditional intravenous patient-controlled analgesia,the effect of ultrasound-guided brachial plexus catheterization for continuous analgesia was also more ideal,with fewer drug side effects and higher patient satisfaction.In view of this,this article now discusses the basic knowledge of ultrasound technology,ultrasound-guided brachial plexus catheterization methods,and commonly used analgesics after brachial plexus catheterization for the use of ultrasound-guided brachial plexus catheterization for postoperative analgesia in upper limb surgery.The research progress is summarized.
