摘要
目的:比较甲氧明与去氧肾上腺素预泵注对剖宫产产妇及新生儿的影响。方法:此为随机对照双盲研究。选择行剖宫产的足月单胎妊娠产妇60例,随机数字表法分成甲氧明组(M组,n=30)、去氧肾上腺素组(P组,n=30)。术前按照标准完成用药的配制。由不知情麻醉医生完成腰—硬联合麻醉。胎儿娩出后,从脐动脉取血行血气分析。结果:甲氧明组产妇收缩压在T8时间点高于去氧肾上腺素组(P<0.05),两组其余时间点血压与心率、血管活性药物用量、不良反应发生、脐动脉血气分析、新生儿Apgar评分比较均无显著差异(P>0.05)。结论:去氧肾上腺素在术中产妇血压、脐动脉血pH值等方面与甲氧明相比无优势,二者均能降低产妇脊髓麻醉后不良反应的发生,并维持新生儿内环境稳定。
Objective:To compare the effects of methoxamine and phenylephrine on puerpera and newborn after cesarean section.Methods:This is a randomized controlled double-blind study.60 full-term singleton pregnant women who underwent cesarean section were randomly divided into methoxamine group(group M,n=30)and phenylephrine group(group P,n=30).The preparation of drugs was completed according to the standard before operation.The combined spinal epidural anesthesia was performed by an unknown anesthesiologist.After delivery,blood was taken from umbilical artery for blood gas analysis.Results:The systolic blood pressure in methoxamine group was higher than that in phenylephrine group at T8 time point(P<0.05).There were no significant differences in blood pressure and heart rate,dosage of vasoactive drugs,adverse reactions,umbilical artery blood gas analysis and neonatal Apgar score between the two groups at other time points(P>0.05).Conclusion:Phenylephrine has no advantage over methoxamine in blood pressure and umbilical artery blood pH during operation.Both of them can reduce the incidence of adverse reactions after spinal anesthesia and maintain the stability of neonatal internal environment.
