摘要
目的:探讨剖宫产术中小剂量应用去氧肾上腺素对产妇血流动力学、循环容量的影响.方法:选取我院2016年3月~2017年12月拟实施腰硬联合麻醉下剖宫产产妇120例,采用随机数字表法分为观察组(麻醉后持续给予0.5μg/kg/min去氧肾上腺素调节血压)、对照组(麻醉后给予等量生理盐水)各60例,对比两组产妇不同时间点的平均动脉压(MAP)、心率(HR)、心输出量(CO)、心脏指数(CI)、产妇血容量、血浆容量、血红蛋白(Hb)、红细胞压积(Hct)及两组的不良反应发生率.结果:T0时刻,观察组和对照组的HR、MAP、CO、CI差异无统计学意义;对照组在T1、T2时刻的HR低于观察组;在T3、T4时刻,对照组的HR高于观察组,CO、CI均低于观察组;对照组在T1、T2、T3时刻的MAP低于观察组,在T4时刻的MAP高于观察组;术前,观察组和对照组的血容量、血浆容量、Hb、Hct差异无统计学意义;术后,观察组的Hb、Hct均高于对照组,观察组的血浆容量均低于对照组;观察组的不良反应发生率6.67%低于对照组的20.00%.结论:小剂量去氧肾上腺素有利于维持剖宫产中术中产妇血流动力学、循环容量的稳定,减轻麻醉相关的不良反应.
Objective To investigate the effect of low dose of phenylephrine on hemodynamics and circulation capacity during cesarean section.Methods In our hospital from March 2016 to December 2017,120 cases of cesarean section of caesarean section under combined spinal and epidural anesthesia were carried out.The random digital table was used to divide the 120 cases into the experimental group(after anesthesia was given 0.5 mu of epinephrine to regulate blood pressure)and the control group(after anesthesia was given equal amount of saline)in 60 cases,compared to the two groups of women at different time points.The incidence of mean arterial pressure(MAP),heart rate(HR),cardiac output(CO),heart index(CI),maternal blood volume,plasma volume,hemoglobin(Hb),hematocrit(Hct),and two groups of adverse reactions were observed.Results At T0 time,there was no significant difference in HR,MAP,CO and CI between the experimental group and the control group,and the control group was lower than the test group at T1 and T2 at the time of T1 and T2,and the control group was lower than the experimental group at the T3 and T4 moments.In the test group.the MAP of the test group was higher than the test group.Before the operation,there was no significant difference in blood volume,plasma volume,Hb,Hct between the experimental group and the control group.The blood volume,Hb and Hct in the test group were higher than those of the control group.The volume of blood plasma in the test group was lower than that of the control group;the incidence of adverse reactions in the experimental group was 6.67%lower than that in the control group(20%).Conclusion Low dose of phenylephrine is beneficial to maintain the stability of hemodynamics and circulatory capacity during cesarean section,and reduce the adverse reactions caused by anesthesia.
