基于CBCT分析下颌第三磨牙萌出状态对下颌第二磨牙牙槽骨缺损的影响 被引量:2

摘要 目的探讨锥束计算机断层摄影(cone beam computed tomography, CBCT)在下颌第三磨牙(mandibular third molars, MTM)萌出状态对下颌第二磨牙(mandibular second molars, MSM)牙槽骨缺损中的评价效果。方法选择兰州大学口腔医院2018年6月至2019年7月下颌阻生MTM合并邻近MSM牙槽骨缺损患者79例作为研究对象,采用CBCT测定咬合平面夹角(occlusal plane angle, AOP)、MTM阻生类型、下颌MTM近中釉牙骨质界面到下颌MSM远中釉牙骨质界的距离(TMCEJ-DCEJ)。查阅患者一般资料,包括性别、年龄,分析不同因素对MSM牙槽骨缺损的影响。结果对于年龄≥65岁患者,远中牙槽骨吸收程度和近中牙槽骨吸收程度差异无统计学意义;25~34岁、35~44岁、45~54岁、55~64岁患者远中牙槽骨吸收程度均大于近中牙槽骨吸收程度(P<0.05);不同年龄段患者远中牙槽骨吸收程度和近中牙槽骨吸收程度差异具有统计学意义(P<0.05);下颌阻生MTM合并邻近MSM牙槽骨缺损79例患者中,轻度43例,中度20例,重度16例。MSM牙槽骨缺损严重程度与年龄、远端牙槽骨吸收(distal alveolar bone resorption, DBR)程度、AOP角度、MTM阻生类型及TMCEJ-DCEJ距离差异具有统计学意义(P<0.05)。结论 MTM萌出状态能影响MSM牙槽骨缺损,对于CBCT检查下AOP角度为11°~79°、MTM处于阻生高位,且TMCEJ-DCEJ在6~9 mm时能增加远中邻面龋坏(distal proximal caries, DPC)及MTM龋坏发生率。 Objective To investigate the effect of the eruption status of mandibular third molars(MTM) on alveolar bone defects of mandibular second molars(MSM) using cone beam computed tomography(CBCT). Methods A total of 79 patients with impacted MTM combined with alveolar bone defect of MSM treated in our hospital from June 2018 to July 2019 were enrolled in this study. Occlusal plane angle(AOP), types of MTM impaction, and cementoenamel junction(CEJ) distances between distal MSMs and mesial MTMs(TMCEJ-DCEJ) were determined by CBCT. The general information of the patients, including gender and age, was also reviewed to analyze the influencing factors on alveolar bone defect of MSM. Results As for the patients aged ≥65 years, there were no significant differences in bone resorption between the distal and mesial alveolar bone. However, the bone resorption of the distal alveolar bone was greater than that of the mesial alveolar bone in the patients at age of 25~34, 35~44, 45~54 and 55~64 years(P<0.05). Significant differences were observed in the bone resorption among different age groups(P<0.05). For the 79 patients with impacted MTM combined with alveolar bone defect of MSM, 43 cases had mild, 20 moderate and 16 severe defects. The severity of alveolar bone defect in MSM was significantly correlated with age, ratio of CEJ-defect distance to tooth root length, AOP, type of MTM impaction and the TMCEJ-DCEJ of the patients(P<0.05). Conclusion The eruption status of MTM can affect the alveolar bone defect of MSM. With the aid of CBCT, AOP of 11°~79°, high impacted MTM, and TMCEJ-DCEJ distance of 6~9 mm can increase the incidence of caries on the distal aspect in MSM and dental caries in MTM.
