摘要
目的:观察调肝化纤丸对慢性乙型肝炎(CHB)肝硬化患者肝功能及远期疗效的影响。方法:选取2018年1月-2018年12月收治的CHB肝硬化患者90例,随机分成观察组和对照组,每组各45例。对照组服用恩替卡韦分散片治疗;观察组在对照组基础上联合调肝化纤丸治疗;疗程均为12个月。分析比较治疗前及治疗后12个月患者肝功能改善情况、肝硬度值(LSM)、HBV-DNA载量及中医证候积分变化等。结果:治疗后,观察组总有效率为95.6%,较对照组的75.6%明显升高(P<0.05);治疗后,观察组患者的中医证候积分、LSM值低于对照组(P<0.05);观察组患者治疗后3、6、12个月的HBV-DNA转阴率均较对照组高(P<0.05);在治疗后12个月,观察组患者HBeAg血清学转换率、HBsAg滴度下降50%均较对照组高(P<0.05)。在治疗后12个月,观察组血清ALB水平高于对照组(P<0.05),ALT、AST及TBil水平低于对照组(P<0.05)。结论:调肝化纤丸辅助治疗CHB肝硬化疗效确切,可改善患者的中医证候积分,减缓肝纤维化进展,促进肝功能恢复,值得临床推广和应用。
Objective:To observe the effect of Tiaogan Huaxian pill on liver function and long-term effect in patients with hepatitis B cirrhosis.Methods:Ninety patients with hepatitis B cirrhosis were randomly divided into the observation group and reference group,45 cases in each group.The reference group was treated with entecavir dispersible tablets;the observation group was treated with Tiaogan Huaxian pill on the basis of the reference group;the course of treatment was 12 months.The improvement of liver function,liver hardness(LSM),HBV-DNA load and TCM syndrome score before and 12 months after treatment were analyzed and compared.Results:After treatment,the total effective rate of the observation group was 95.6%,which was higher than 75.6%of the reference group(P<0.05);after treatment,the TCM syndrome integral and LSM value of the observation group were lower than that of the reference group(P<0.05);the negative rate of HBV-DNA in 3,6 and 12 m was higher in the observation group than that in the reference group(P<0.05).At the end of 12 m,HBeAg seroconversion rate and HBsAg titer decreased by 50%significantly higher than those in the reference group(P<0.05).At the end of 12 m,the level of ALB was higher in the observation group than that in the reference group(P<0.05),and the levels of ALT,AST and TBil were lower in the observation group than those in the reference group(P<0.05).Conclusion:Tiaogan Huaxian pill is effective in the treatment of hepatitis B cirrhosis.It can improve the TCM syndrome score of the patients,slow down the process of liver fibrosis,improving liver function.It is worthy of promotion and clinical application.
