摘要
目的依据闭合复位外侧运用交叉克氏针相关张力带原理实施固定手术治疗干预对策用于内侧柱塌陷型肱骨髁上骨折儿童临床治疗中的意义。方法针对本医院25例儿童闭合复位外侧运用交叉克氏针相关张力带原理实行固定手术治疗干预,固定手术治疗之前及固定手术治疗之后即刻Baumann角测量度数、随访总体时长、骨折愈合所用时长、最后一次随访时肘关节活动具体范围情况、肘关节功能恢复情况、固定手术治疗之后并发症情况。结果手术治疗后Baumann角测量度数相比固定手术治疗之前减小(P<0.05);随访总体时长(18±3)个月,骨折愈合所用时长(4.9±0.5)周;屈曲角度(129.4°±10.2°),伸直角度均数值(15.1°±2.3°),旋前角度(78.1°±3.3°),旋后角度(78.4°±4.1°);肘关节功能恢复优良情况88%;无严重并发症出现。结论儿童内侧柱塌陷型肱骨髁上骨折闭合复位外侧运用交叉克氏针相关张力带原理开展固定手术治疗干预显示较佳固定效果。
Objective Based on the closed reduction and lateral application of the principle of cross-kirschner Wire related tension band to implement the intervention strategy of surgical fixation for the clinical treatment of children with medial column collapse supracondylar fracture of humerus.Methods Tenty-five cases of this hospital children closed reduction and tension with the outside by the needle was cross correlation principle fixed surgical intervention,fixed before the surgery and fixed immediately after surgery Baumann measuring angles,the follow-up for overall length,fracture healing time,the specific scope of elbow movement at the time of final follow-up,elbow joint function recovery,fixed complications after surgical treatment.Results Baumann Angle measurement degree after surgical treatment was reduced compared with that before fixed surgery(P<0.05).The mean total follow-up time was(18±3)months,and the mean fracture healing time was(4.9±0.5)weeks.The average value of buckling Angle(129.4°±10.2°),straightening Angle(15.1°±2.3°),pre-rotation Angle(78.1°±3.3°),post-rotation Angle(78.4°±4.1°);88%of the elbow joint function recovered well.No serious complications occurred.Conclusion The closed reduction of children′s medial column collapsed supracondylar fractures of the humerus by closed reduction and lateral fixation with the principle of cross Kirschner wires related tension bands shows a better fixation effect.
