Clinical effect ofα-chymotrypsin aerosol inhalation in the treatment of chronic obstructive pulmonary disease complicated with community acquired pneumonia

摘要 目的探究慢性阻塞性肺疾病(COPD)合并社区获得性肺炎(CAP)患者使用α-糜蛋白酶雾化吸入的治疗效果。方法80例COPD合并CAP患者,随机分为研究组与对照组,各40例。对照组患者接受常规治疗,研究组在对照组治疗基础上接受α-糜蛋白酶雾化吸入治疗。对比两组患者治疗前后痰液量、动脉血氧分压(PaO2)及第1秒用力呼气容积占预计值的百分比(FEV1%pred),治疗后痰液粘稠度,住院时间。结果治疗后,研究组痰液量为(3.21±1.46)ml,明显少于对照组的(5.02±1.53)ml,差异具有统计学意义(P<0.05)。治疗后,研究组患者PaO2和FEV1%pred分别为(115.69±5.78)mm Hg(1 mm Hg=0.133 kPa)、(73.58±10.12)%,均明显高于对照组的(98.64±3.75)mm Hg、(60.48±9.21)%,差异具有统计学意义(P<0.05)。治疗后,对照组患者痰液粘稠度Ⅰ度16例、Ⅱ度18例、Ⅲ度6例;研究组患者痰液粘稠度Ⅰ度27例、Ⅱ度11例、Ⅲ度2例。研究组患者痰液粘稠度优于对照组,差异具有统计学意义(P<0.05)。研究组患者住院时间为(6.40±2.75)d,明显短于对照组的(10.59±3.48)d,差异具有统计学意义(P<0.05)。结论对COPD合并CAP患者采用α-糜蛋白酶雾化吸入治疗能有效改善其痰液粘稠度,促进气道痰液排出,减少肺部痰液量,改善机体血氧水平及肺功能,并缩短患者住院时间,具有良好的临床应用效果。 Objective To investigate the therapeutic effect ofα-chymotrypsin aerosol inhalation in the treatment of chronic obstructive pulmonary disease(COPD)complicated with community acquired pneumonia(CAP).Methods A total of 80 cases of COPD complicated with CAP were randomly divided into research group and control group,with 40 cases in each group.The control group was treated with conventional therapy,and the research group was treated withα-chymotrypsin aerosol inhalation based on the control group.The sputum volume,arterial blood oxygen partial pressure(PaO2)and forced expiratory volume in the first second as percentage of predicted value(FEV1%pred)before and after treatment,sputum viscosity after treatment,and hospitalization time were compared between the two groups.Results After treatment,the sputum volume(3.21±1.46)ml of the research group was obviously lower than(5.02±1.53)ml of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of PaO2 and FEV1%pred of the research group were(115.69±5.78)mm Hg(1 mm Hg=0.133 kPa)and(73.58±10.12)L,which were obviously higher than(98.64±3.75)mm Hg and(60.48±9.21)L of the control group,and the difference was statistically significant(P<0.05).After treatment,the control group had gradeⅠof sputum viscosity in 16 cases,gradeⅡin 18 cases,and gradeⅢin 6 cases;the research group had gradeⅠof sputum viscosity in 27 cases,gradeⅡ11 cases,and gradeⅢ2 cases.The sputum viscosity of the research group was better than that of the control group,and the difference was statistically significant(P<0.05).The hospitalization time(6.40±2.75)d of the research group was obviously shorter than(10.59±3.48)d of the control group,and the difference was statistically significant(P<0.05).Conclusionα-chymotrypsin aerosol inhalation therapy can effectively improve the sputum viscosity,promote the airway sputum discharge,reduce the lung sputum volume,improve the blood oxygen level and lung function,and shorten the hospitalization time of CO
