肺表面活性物质加持续气道正压通气治疗NRDS的应用及有效性分析

摘要 目的分析肺表面活性物质加持续气道正压通气治疗NRDS的应用及有效性。方法简单随机选择该院2018年11月—2019年11月NRDS患儿共100例,数字表随机分两组,每组50例,对照组的患儿给予持续气道正压通气治疗,观察组在该基础上增加肺表面活性物质。比较两组住院时间、治疗前后患儿血气指标动脉血氧分压、二氧化碳分压、总有效率、不良反应。结果观察组住院时间(8.13±2.21)d短于对照组(11.25±3.01)d,差异有统计学意义(P<0.05)。治疗前二组患儿血气指标动脉血氧分压、二氧化碳分压比较,差异无统计学意义(P>0.05),而治疗后两组血气指标动脉血氧分压、二氧化碳分压均改善,而观察组血气指标动脉血氧分压(84.21±7.52)mmHg显著高于对照组(76.21±6.78)mmHg,二氧化碳分压(31.21±3.55)mmHg显著低于对照组(34.21±4.89)mmHg,差异有统计学意义(t=5.944、7.834,P<0.05)。观察组总有效率96.00%高于对照组76.00%(χ^(2)=8.306,P=0.004<0.05)。两组治疗过程不良反应未见。结论持续气道正压通气联合肺表面活性物质对于NRDS的治疗效果确切。 Objective To analyze the application and effectiveness of pulmonary surfactant plus continuous positive airway pressure in the treatment of NRDS.Methods A total of 100 children with NRDS in the hospital from November 2018 to November 2019 were selected and randomly divided into 2 groups with 50 patients in each group.Children in the control group were given continuous positive airway pressure(CPAP)therapy,and pulmonary surfactant was added in the observation group on the basis of this.The length of hospital stay,partial arterial blood oxygen pressure,partial arterial blood carbon dioxide pressure,total effective rate and adverse reactions were compared between the two groups.Results The length of hospital stay in the observation group(8.13±2.21)d was shorter than that in the control group(11.25±3.01)d,the difference was statistically significant(P<0.05).Two groups before treatment in children with blood gas index of arterial blood oxygen partial pressure,PaCO_(2) partial pressure,the difference was not statistically significant(P>0.05),while the two groups after treatment blood gas index of arterial blood oxygen partial pressure,PaCO_(2) partial pressure are improved,and the observation group blood gas index of arterial blood oxygen partial pressure(84.21±7.52)mmHg is significantly higher than control group(76.21±6.78)mmHg,PaCO2 partial pressure(31.21±3.55)mmHg was significantly lower than the control group(34.21±4.89)mmHg,the difference was statistically significant(t=5.944,7.834,P<0.05).The total effective rate in the observation group was 96.00% higher than that in the control group 76.00%,the difference was statistically significant(χ^(2)=6.845,P=0.012<0.05).No adverse reactions were observed during treatment in the two groups.Conclusion Continuous positive airway pressure combined with pulmonary surfactant has a definite therapeutic effect on NRDS.
