摘要
目的观察益肾活血汤结合平衡针灸治疗原发性高血压病(气虚血瘀证)的临床疗效及安全性研究。方法选择2018年8月—2019年8月医院收治的原发性高血压患者200例,依据随机对照法将患者分为研究组和对照组,每组100例。对照组患者采取西医常规治疗,口服左旋氨氯地平治疗。研究组在对照组治疗的基础上,口服益肾活血汤结合平衡针灸治疗。益肾活血汤1剂/d,水煎后分早晚2次服用,平衡针灸取穴:降压穴,每天1次。两组均持续治疗8周。比较两组治疗前后眩晕头重、胸闷恶心、纳呆痰多和身疲力乏等症候积分和治疗疗效,比较治疗前后24 h ABP水平,比较治疗前后ACE、IGF-1和AngⅡ水平和脑动脉平均低速血流速度,观察两组治疗过程中的不良反应情况。结果研究组治疗总有效率为96.00%(96/100)高于对照组治疗总有效率的88.00%(88/100)(P<0.05);研究组治疗后眩晕头重、胸闷恶心、纳呆痰多和身疲力乏等症候积分低于对照组(P<0.05);研究组治疗后24 h MSBP和24 h MSDP水平低于对照组(P<0.05);研究组治疗后ACE、IGF-1和AngⅡ水平低于对照组(P<0.05);研究组治疗后脑动脉MCA、VA和BA平均低速血流速度高于对照组(P<0.05)。结论益肾活血汤和平衡针灸结合治疗原发性高血压疗效确切,可有效的降低血压,改善患者的临床症状,调节患者颅内动脉血流速度,降低血清中ACE、IGF-1和AngⅡ水平,提高治疗疗效,值得临床借鉴参考。
Objective To observe the clinical efficacy and safety of Yishen Huoxue Decoction(益肾活血汤)combined with balanced acupuncture in the treatment of essential hypertension(Qi deficiency and blood stasis syndrome).Methods A total of 200 patients with essential hypertension admitted to our hospital from August 2018 to August 2019 were selected,and the patients were divided into study group and control group according to the randomized control method,with 100 patients in each group.The control group was treated with conventional western medicine and oral amlodipine.On the basis of the treatment of the control group,the study group received oral Yishen Huoxue Decoction combined with balanced acupuncture and moxibustion.Yishen Huoxue Decoction was given 1 dose/d,twice a day,taking in the morning and evening.The balanced acupuncture selected the reducing pressure point,once a day.Both groups continued treatment for 8 weeks.The two groups’scores of vertigo,heavy head,chest tightness,phlegm and body fatigue and treatment curative effect,symptom integral 24 hABP level,levels of ACE,IGF 1 and AngⅡand average cerebral artery blood flow velocity at low speed were compared before and after treatment.And We observed adverse reactions in two groups.Results The total effective rate in the study group was 96.00%(96/100),higher than that in the control group(P<0.05).After treatment,the scores of symptoms such as dizziness,heavy head,chest tightness and nausea,natrium,phlegm and body fatigue were lower in the study group than those in the control group(P<0.05).After treatment,the levels of 24 h MSBP and 24 h MSDP in the study group were lower than those in the control group(P<0.05).After treatment,levels of ACE,IGF 1 and AngⅡwere lower than those of the control group(P<0.05).After treatment,the mean low blood flow velocity of MCA,VA and BA in the study group was higher than that in the control group(P<0.05).Conclusion Yishen Huoxue Decoction and balanced acupuncture and moxibustion treatment of essential hypertension can e
