摘要
目的探讨外侧小切口髋关节置换术(MIS-THA)与后路小切口髋关节置换术(PMIHR)治疗老年创伤性股骨颈骨折临床对照研究。方法选定该院于2017年2月—2019年2月收治的168例老年创伤性股骨颈骨折患者,机械取样法分为观察组(84例,PMIHR)与对照组(84例,MIS-THA)两组,比较组间老年创伤性股骨颈骨折患者的引流量、手术出血量、止痛药用量、住院天数、Harris评分及VRS评分。结果观察组老年创伤性股骨颈骨折患者的引流量(152.38±11.74)mL、手术出血量(148.62±15.47)mL、止痛药用量(10.73±1.38)g,均较对照组低,差异有统计学意义(t=19.785、8.983、34.526,P<0.05);其住院天数(12.39±1.63)d,较对照组短,差异有统计学意义(t=15.557,P<0.05)。观察组患者手术后Harris评分(85.31±8.63)分,较对照组高,差异有统计学意义(t=5.957,P<0.05);其手术后VRS评分(1.28±0.25)分手术后,较对照组低,差异有统计学意义(t=13.957,P<0.05)。结论PMIHR有助于减少老年创伤性股骨颈骨折患者的引流量与手术出血量,并可对其髋关节功能进行改善。
Objective To explore a clinical controlled study of lateral small incision hip replacement(MIS-THA)and posterior small incision hip replacement(PMIHR)in the treatment of elderly traumatic femoral neck fractures.Methods A total of 168 elderly patients with traumatic femoral neck fractures admitted to the hospital from February 2017 to February 2019 were selected.The mechanical sampling method was divided into observation group(84 cases,PMIHR)and control group(84 cases,MIS-THA),the drainage volume,surgical blood loss,painkiller dosage,length of hospital stay,Harris score and VRS score of elderly patients with traumatic femoral neck fracture between the groups were compared.Results The drainage volume of elderly patients with traumatic femoral neck fracture in the observation group was(152.38±11.74)mL,the amount of surgical bleeding(148.62±15.47)mL,and the dosage of analgesics(10.73±1.38)g were lower than those in the control group,the difference was statistically significant(t=19.785,8.983,34.526,P<0.05);their hospitalization days were(12.39±1.63)days,which was shorter than the control group,the difference was statistically significant(t=15.557,P<0.05).The Harris score(85.31±8.63)points of the observation group after surgery was higher than that of the control group,the difference was statistically significant(t=5.957,P<0.05);its VRS score after surgery(1.28±0.25)points was lower than that of the control group,the difference was statistically significant(t=13.957,P<0.05).Conclusion PMIHR can help reduce drainage and surgical bleeding in elderly patients with traumatic femoral neck fractures,and can improve their hip joint function.
