摘要
目的股前外侧嵌合皮瓣与串联皮瓣修复口腔颌面部肿瘤根治术后缺损的临床分析。方法方便选取2018年12月—2019年12月来该院的76例口腔颌面部肿瘤根治术后缺损患者作为研究对象,按照入院先后顺序将患者分为实验组与对照组,每组38例。实验组患者采用股前外侧嵌合皮瓣修复,对照组患者采用串联皮瓣修复,修复后评价临床治疗效果、术后缺损修复情况、不良反应发生率。结果实验组手术时间(6.01±1.19)h、皮瓣制作时间(1.15±0.26)h及胃管拔除时间(19.02±3.35)d相较对照组明显偏低,差异有统计学意义(t=6.433、4.666、3.347,P<0.05)。实验组术后口腔闭合功能(8.67±0.35)分、咀嚼功能(8.89±0.53)分、进食情况(8.58±0.47)分及吞咽功能(8.61±0.57)分均明显优于对照组,差异有统计学意义(t=5.976、4.435、3.289、4.917,P<0.05)。实验组不良反应发生率5.26%相较对照组23.68%明显偏低,差异有统计学意义(χ^(2)=5.208,P=0.022)。结论对口腔颌面部肿瘤根治术后缺损患者而言,股前外侧嵌合皮瓣修复效果相较串联皮瓣更为突出,不但能改善术后缺损修复情况,又能确保治疗安全性。
Objective Clinical analysis of anterolateral femoral chimeric flaps and tandem flaps to repair defects after radical resection of oral and maxillofacial tumors.Methods From December 2018 to December 2019,conveniently select 76 patients with defects after radical resection of oral and maxillofacial tumors in the hospital will be used as the research object.The patients were divided into experimental group and control group according to the order of admission.The anterolateral thigh chimeric flap was repaired,and the control group was repaired with tandem flaps.After the repair,the clinical treatment effect,postoperative defect repair,and adverse reaction rate were evaluated.Results The operation time of the experimental group was(6.01±1.19)h,the time of flap making was(1.15±0.26)h and the time of gastric tube removal was(19.02±3.35)d,which were significantly in the experimental group lower than those of the control group,and the difference was statistically significant(t=6.433,4.666,3.347,P<0.05).The oral closure function(8.67±0.35)points,chewing function(8.89±0.53)points,eating condition(8.58±0.47)points and swallowing function(8.61±0.57)points of the experimental group after operation were significantly better than those of the control group,and the difference was statistically significant(t=5.976,4.435,3.289,4.917,P<0.05).The adverse reaction rate of 5.26%in the experimental group was significantly lower than that of 23.68%in the control group,and the difference was statistically significant(χ^(2)=5.208,P=0.022).Conclusion For patients with defects after radical resection of oral and maxillofacial tumors,the repair effect of the anterolateral femoral chimeric flap is more prominent than that of the tandem flap.It can not only improve the repair of the defect after surgery,but also ensure the safety of treatment.
