摘要
目的探讨肌钙蛋白I对危重症患者预后的评估价值。方法方便选取2014年1月—2015年12月该院收治的126例综合ICU患者,根据肌钙蛋白I(cTnI)阳性和阴性将患者分为两组,对两组患者的临床指标进行单因素和多变量Logistic分析。结果126例患者中cTnI阳性组患者50例(A组),阴性者76例(B组),单因素分析A、B两组患者的年龄≥65岁、APACHEⅡ评分≥15分、血液净化治疗、机械通气治疗、低蛋白血症、MODS所占比例显著高于B组,两组差异有统计学意义(χ^(2)=5.305、6.603、18.582、6.172、4.919、8.020,P<0.05)。Logistic回归分析表明cTnI阳性(OR=26.41,P<0.05)、年龄≥65岁(OR=8.30,P=0.017)、APACHEⅡ评分≥15(OR=6.13,P=0.005)、血液净化治疗(OR=5.18,P=0.017)以及MODS(OR=23.80,P=0.001)是影响患者预后的危险因素。结论cTnI对危重症患者的预后评估具有较大的临床价值。
Objective To investigate the prognostic value of troponin I in critically ill patients.Methods conveniently select 126 patients admitted to the comprehensive ICU of the hospital from January 2014 to December 2015 were divided into two groups according to the positive and negative troponin I(cTnI).The clinical indicators of the two groups were single-factor and multivariate Logistic analysis.Results 126 patients including 50 patients in the cTnI positive group(Group A)and 76 patients in the negative group(Group B).The proportion age of patients in the AB group was≥65 years,APACH II score≥15 points,blood purification therapy,mechanical ventilation therapy,hypoproteinemia,the of MODS were significantly higher than that of group B,and the difference between the two groups was statistically significant(χ^(2)=5.305,6.603,18.582,6.172,4.919,8.020,P<0.05).Logistic regression analysis showed that cTnI was positive(OR=26.41,P<0.05),age≥65 years(OR=8.30,P=0.017),APACHEⅡscore≥15(OR=6.13,P=0.005),blood purification treatment(OR=5.18,P=0.017)and MODS(OR=23.80,P=0.001)were risk factors affecting the prognosis of patients.Conclusion cTnI has great clinical value for the prognosis assessment of critically ill patients.
