摘要
目的对比分析快速染色法与革兰氏染色法在阴道分泌物涂片检查中的应用效果及在阴道炎诊断中的价值。方法回顾性选取北京妇产医院2019年10月至2019年11月疑诊的阴道炎患者120例作为研究对象(其中116例确诊为阴道炎),以无菌棉拭子采集患者阴道分泌物并制作成2组样本,分别进行快速染色法与革兰氏染色法检测。根据临床症状、体征、实验室结果做出临床诊断。统计快速染色法与革兰氏染色法对病原体检出的数量、灵敏度、特异性、准确率及两种方法的经济成本与效率,分析它们之间的差异。结果120例患者中,革兰氏染色法与快速染色法分别检出病原体101、97例,其中线索细胞分别为52、48例,真菌分别为30、31例,滴虫均为2例,混合感染分别为17、16例,2种方法各病原体检出率差异无统计学意义(均P>0.05);且120例疑诊阴道炎患者经确诊后,阴道炎116例,非阴道炎4例。快速染色法与革兰氏染色法检出阴道炎分别为97与101例,非阴道炎分别为23与19例,与确诊结果一致数分别为101与105例;同时2种检测方法病原体检出的灵敏度、特异性、准确率比较,差异均无统计学意义(均P>0.05);且快速染色法的经济成本更低,操作时长更短。结论快速染色法检测结果准确、灵敏,特异性好,且与革兰氏染色法检测结果对比差异无统计学意义,但经济成本更低,操作时长更短,效率更高。
Objective To compare and analyze the application effect of rapid staining method and Gram staining method in vaginal secretion smear examination and its value in diagnosis of vaginitis.Methods We retrospectively selected 120 patients with suspected vaginitis from October,2019 to November,2019 in Beijing Obstetrics and Gynecology Hospital as the research objects(of which 116 cases were diagnosed with vaginitis).The sterile cotton swab was used to collect the vaginal secretions of all the patients and the subjects were divided into 2 groups,which were tested by either rapid staining or Gram staining.According to the clinical symptoms and experimental results,we the made clinical diagnosis among these patients.The sensitivity,specificity,summary numbers,accuracy,the cost and efficiency were all obtained to analyze the difference between the two methods.Results Among 120 patients,101 and 97 pathogens were detected by Gram staining and rapid staining,respectively.There were 52 and 48 clue cells,30 and 31 molds,2 trichomonas,17 and 16 mixed infections,and there was no significant difference in the detection rate of pathogens between the 2 methods(all P>0.05).In addition,After 120 cases of suspected vaginitis were diagnosed,116 cases had vaginitis and 4 cases had non-vaginitis.97 and 101 cases of vaginitis were detected by rapid staining and Gram staining,and 23 and 19 cases of non-vaginitis,respectively,and the number consistent with the confirmed results were 101 and 105 cases,respectively.The sensitivity,specificity and accuracy rate of pathogens detected by the 2 detection methods were not significantly different(all P>0.05).And the cost of rapid staining was lower,and its operation time was shorter.Conclusion The results of rapid staining method are accurate,sensitive and specific,and there is no significant difference with the results of Gram staining method.This method has advantages of lower cost,shorter operation time and is more convenient.
