摘要
目的了解医院保肝药的临床使用情况,并且对抽取的794张处方进行分析,为临床使用提供合理依据,为患者提供更合理的用药方案。方法抽取医院2019年1—12月门、急诊保肝药处方794张(急诊60张,门诊734张)。研究涉及的保肝药共分为5大类10个品种。分析各个科室保肝药的使用情况,对保肝药的适应证、用法用量、联合用药等进行一个综合性的分析,判断临床用药是否合理,并对不合理的情况进行探讨。结果进行分析的794张保肝药处方中:抗氧化剂/降酶药占比35%,促肝细胞修复药占比30%,中药制剂占比26%,抗炎保肝药占比5%,解毒保肝药占比4%。保肝药使用例数前三位的科室是消化内科、内分泌科和血液风湿免疫科。不合理用药主要为三种:选药不合理11例、3例用药存在禁忌证,给药剂量不合理82例。结论不同保肝药在不同科室的使用情况不太一样,主要取决于各类保肝药的作用机制和患者的自身情况。通过对各个科室处方的分析,医院保肝药物的使用情况基本趋于合理,存在部分不合理现象,对不合理情况进行分析总结,应制定相应的标准,加强临床用药合理规范。
Objective To understand the clinical use of hepatoprotective drugs in our hospital,and analyze 794 prescriptions,so as to provide a reasonable basis for clinical use and a more reasonable medication plan for patients.Methods 794 prescriptions of hepatoprotective drugs(60 in emergency department and 734 in outpatient department)were selected from January to December 2019.The hepatoprotective drugs involved in the study are divided into 5 categories and 10 varieties.Objective to analyze the use of hepatoprotective drugs in various departments,make a comprehensive analysis on the indications,usage and dosage,combined use of hepatoprotective drugs,judge whether the clinical medication is reasonable,and discuss the unreasonable situation.Results Among the 794 prescriptions for liver protection,the antioxidant/enzyme-lowering drug accounted for 35%,the hepatocyte repairing drug accounted for 30%,the traditional Chinese medicine preparation accounted for 26%,and the anti-inflammatory and hepatoprotective drug accounted for 5%.Hepatic medicine accounted for 4%.The top three departments for the use of liver-protecting drugs are the department of gastroenterology,endocrinology and blood rheumatology.There are three main types of irrational use:11 cases of unreasonable drug selection,3 cases of contraindications,and 82 cases of unreasonable dosage.Conclusion The use of different types of hepatoprotective drugs in different departments is not the same,mainly depends on the mechanism of action of various types of liver-protecting drugs and the patient's own situation.Through the analysis of the prescriptions of various departments,the use of liverprotecting drugs in this hospital is basically reasonable,and there are some unreasonable phenomena.To analyze and summarize the unreasonable situation,corresponding standards should be formulated to strengthen the rational regulation of clinical medication.
