摘要
目的探讨MRI压脂技术在膝关节患者中的应用效果及影像学特点。方法选择2018年5月至2020年1月膝关节损伤患者89例为对象,所有患者均经手术组织检查确诊,确诊前患者均行CT、MRI压脂技术检查,并将其与手术组织检查结果进行比较,分析其影像学特点;绘制ROC曲线,分析MRI压脂技术、CT在膝关节患者中的诊断效能(灵敏度、特异度)。结果 89例膝关节损伤患者均经手术检测确诊,其中骨折32例,关节软骨损伤/骨挫伤36例,半月板损伤6例,韧带损伤9例,关节腔积液6例;CT检查确诊75例,诊断符合率为84.27%(P<0.05);MRI压脂技术确诊85例,诊断符合率为95.51%(P>0.05);MRI压脂技术中膝关节内侧副韧带形态异常32例,明显增粗35例,迂曲挛缩22例;ROC曲线结果表明:MRI压脂技术联合CT用于膝关节损伤患者中诊断灵敏度高于单一MRI压脂技术、CT检查(P<0.05);特异度低于单一MRI压脂技术、CT检查(P<0.05)。结论 MRI压脂技术用于膝关节损伤患者中能获得较高的诊断检出率,联合CT检测患者诊断效能较高,能为临床诊疗提供影像学依据。
Objective To investigate the application effect and imaging characteristics of MRI fat compression technique in knee joint patients. Methods From May 2018 to January 2020, 89 patients with knee joint injuries were selected as subjects. All patients were diagnosed by surgical tissue examination. Before the diagnosis, the patients were examined by CT and MRI liposuction techniques, and they were examined with surgical tissue. The results were compared, and the imaging characteristics were analyzed;ROC curve was drawn to analyze the diagnostic efficiency(sensitivity, specificity) of MRI fat pressure technology and CT in knee patients. Results Eighty-nine cases of knee joint injuries were diagnosed by surgical examination, including 32 cases of fractures, 36 cases of articular cartilage injury/bone contusion, 6 cases of meniscus injury, 9 cases of ligament injury, 6 cases of joint cavity effusion;CT examination confirmed 75 cases The diagnosis coincidence rate was 84.27%(P<0.05);85 cases were diagnosed by MRI fat pressure technique, and the diagnosis coincidence rate was 95.51%(P>0.05);32 cases of MRI fat pressure technique were abnormal in the shape of the medial collateral ligament of the knee, which was obvious 35 cases of thickening, 22 cases of tortuous contracture;ROC curve results show that the diagnostic sensitivity of MRI fat pressure technology combined with CT in patients with knee joint injury is higher than that of single MRI fat pressure technology and CT examination(P<0.05);the specificity is lower single MRI fat pressure technique, CT examination(P<0.05). Conclusion MRI liposuction technology can obtain a high diagnostic detection rate in patients with knee joint injuries, and the combined CT detection has a high diagnostic efficiency. It can provide imaging evidence for clinical diagnosis and treatment and is worthy of popularization and application.
