摘要
【目的】探讨血清前白蛋白(PA)与系统性红斑狼疮(SLE)活动度和早期肾损伤的相关性。【方法】恨据SLEDAI评分将103例ShE患者分为轻度活动组(42例)、中度活动组(25例)、重度活动组(36例);同时根据肾小球滤过率(eGFR)将患者分为肾功能正常组(49例)、肾功能异常组(54例)。收集患者年龄、身高、体质量、收缩压(SBP)、舒张压(DBP)、血小板(PLT)、血红蛋白(Hb)、血白蛋白(ALB)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、超敏C反应蛋白(hs-CRP)、血肌酐(SCr)、血尿素氮(BUN)、血尿酸(UA)、p2微球蛋白(β2MG)、24 h尿蛋白定量(UTP)、血沉(ESR)、补体C3(C3)、补体C4(C4)及血清PA。采用二分类logistic回归分析肾功能损伤的危险因素,分析PA与上述危险因素的相关性。采用ROC曲线评估血清PA预测肾功能损伤的价值。【结果】(1)PA水平在不同SLEDAl评分轻度活动组最高.中度活动组次之,重度活动组最小(P<0.05)。(2)PA水平在肾功能正常组为(212.34±60.92)mg/L,明显高于异常组的(178.23±62.09)mg/L(P<0.05)。(3)肾功能下降的独立危险因素是年龄、UA及Hb。Pearson相关分析结果显示PA与年龄呈正相关(P<0.05).与UA及Hb呈负相关(均P<0.05)。(4)ROC评估PA预测肾功能损伤的曲线下面积(AUC)为0.764,灵敏度、特异度分别为67.2%和72.9%。【结论】PA与SLE活动度和肾损伤具有相关性,可作为评价SLE活动度和肾损伤的指标之一。
[Objective]To investigate the correlation between serum prealbumin(PA)and systemie lupus ery-thematosus(SLE)activity and early renal injury.[Methods]According to SLEDAI score.103 SLE patients were divided into mild activity group(42 cases),moderate activity group(25 cases)and severe activity group(36 ca-ses);meanwhile,according to glomerular filtration rate(EGFR),the patients were divided into normal renal funetion group(49 cases)and abnormal renal function group(54 cases).Age.height,body mass,systolie blood pressure(SBP).diastolie blood pressure(DBP),platelet(PLT),hemoglobin(Hb),serum albumin(ALB),to-tal cholesterol(TC),triglyeride(TG),low density lipoprotein(L.DL-C),hith density lipoprotein(HDL-C),high sensitivity C-reactive protein(hs-CRP),serum creatinine(SCr).blood urea nitrogen(BUN),serum uric acid(UA),2 microglobulin(2MG),24 h urinary protein(UTP).erythroeyte sedimentation rate(ESR),com-plement C3(C3),complement C4(C4)and serum PA were ollected.Binary logistie regression was used to ana-lyze the risk factors of renal funection injury.and the correlation between PA and the above risk factors was ana-lyzed.ROC curve was used t0 evaluate the value of serum PA in predicing renal funetion injury.[Results](1)In different SLEDAI scores.PA level was the highest in mild activity group.followed by the moderate activity group.and the lowest in the severe activity group(P<0.05).(2)The level of PA in the normal renal function group was(212.34±60.92)mg/L.which wss signifieantly higher than that in the abnormal renal function group(178.23±62.09)mg/L(P<0.05).(3)Age,UA and Hb were independent risk factors for renal dysfunction.Pearson correlation analysis showed that PA was positively correlated with age(P<0.05),and negatively correlated with UA and Hb(P<0.05).(4)The area under the curve(AUC)of ROC was 0.764.the sensitivity and specificity were 67.2%and 72.9%respectively.[Conclusion]PA is correlated with SLE activity and renal injury,and can be used as one of the indexes to evaluate SLE activity and renal injury.
