摘要
目的 探讨阿加曲班联合普伐他汀对急性脑梗死患者外周血CD133+细胞、血浆巯基丙酮酸硫基转移酶水平的影响.方法 将90例急性脑梗死患者按随机数字表法分为两组,各45例.两组均予以常规治疗,研究组在此基础上予以阿加曲班联合普伐他汀治疗,治疗28 d.统计并比较两组临床疗效有效率、不良反应发生率,治疗前及治疗后第14 d美国国立卫生研究院卒中量表评分、Barther指数量表及外周血CD133+细胞、血浆巯基丙酮酸硫基转移酶、血清炎性因子(白细胞介素-6、超敏C反应蛋白、肿瘤坏死因子-α)水平.结果 研究组总有效率显著高于对照组(P<0.05).治疗后第14 d,两组美国国立卫生研究院卒中量表评分、白细胞介素-6、超敏C反应蛋白、肿瘤坏死因子-α水平较治疗前显著降低(P<0.01),研究组显著低于对照组(P<0.01);两组Barther指数、CD133+细胞、血浆巯基丙酮酸硫基转移酶水平较治疗前显著升高(P<0.01),研究组显著高于对照组(P<0.01).两组不良反应发生率比较差异无统计学意义(P>0.05).结论 阿加曲班联合普伐他汀治疗急性脑梗死疗效显著,可明显改善患者的神经功能及日常生活活动能力,有效增加外周血CD133+细胞及血浆巯基丙酮酸硫基转移酶水平,降低炎性因子水平.
Objective To explore the effects of argatroban combined with pravastatin on peripheral blood CD133^(+)cells and plasma 3-MST expression in patients with acute cerebral infarction.Methods 90 pa-tients with acute cerebral infarction were divided into two groups according to the random number table method,with 45 cases in each group.Both groups were given conventional treatment,and the study group was given argatroban combined with pravastatin on this basis for 28 days.The clinical efficacy,incidence of adverse reactions and NIHSS,Barther index,CD133+cells in peripheral blood,plasma 3-MST,serum inflammatory factors(IL-6、hs-CRP.TNF-α)levels were compared between the two groups before and 14 days after treatment.Results The total effective rate of the study group was significantly higher than that of the control group(P<0.05).At the end of 14 days after treatment,the levels of NIHSS,IL-6,hs-CRP,and TNF-α in the two groups were significantly lower than before treatment(P<0.01),and the study group was significantly lower than the control group(P<0.01).The Barther index,CD133+cells.and plasma 3-MST levels in the two groups were significantly higher than before treatment(P<0.01),and the study group was significantly higher that the control group(P<0.01).There was no sig-nificant difference in the incidence of adverse reac-tions between the two groups(P>0.05).Conclu-sions Argatroban com bined with pravastatin has a significant effect in the treatment of acute cerebral infarction.It can significantly improve the patient's neu-rological function and activities of daily living,effectively increase the levels of CD133^(+)cells and plasma 3-MST in the peripheral blood,and reduce the levels of inflammatory factors.
