摘要
目的探讨中性粒细胞计数/淋巴细胞计数(NLR)、血小板计数/淋巴细胞计数(PLR)联合CA125对上皮性卵巢癌诊断及预后评估的意义。方法收集青岛大学附属医院2010年1月至2015年12月收治的236例卵巢上皮性癌及100例卵巢良性肿瘤的病例资料。通过ROC分析,得出NLR、PLR联合CA125对诊断上皮性卵巢癌的灵敏度和特异性。通过单因素及COX回归模型分析对生存期的影响。结果低NLR组与高NLR组、低PLR组与高PLR组在FIGO分期、组织分化、腹水、CA125、肿瘤细胞减灭术满意程度、术后复发上差异有统计学意义(P<0.05)。NLR、PLR联合CA125诊断上皮性卵巢癌灵敏度及特异度更高。年龄≥50岁、NLR≥2、PLR≥150、Ⅲ~Ⅳ期、G3、淋巴结转移、肿瘤细胞减灭术满意程度是影响预后的危险因素。PLR、FIGO分期、肿瘤细胞减灭术满意程度是影响总生存期的独立危险因素。结论NLR、PLR联合CA125可以作为上皮性卵巢癌诊断及评估预后的敏感指标,高NLR、高PLR提示预后不良。
Objective To investigate the significance of neutrophil/lymphocyte count(NLR),platelet/lymphocyte count(PLR)combined with CA125 in the diagnosis and prognosis evaluation of epithelial ovarian cancer.Methods Data of 236 cases of epithelial ovarian cancer and 100 cases of benign ovarian tumor were collected from January 2010 to December 2015 in Affiliated Hospital of Qingdao University.Through ROC analysis,the sensitivity and specificity of NLR,PLR combined with CA125 in the diagnosis of epithelial ovarian cancer were obtained.Univariate and Cox regression models were used to analyze the influence on the survival.Results There were significant differences in FIGO staging,tissue differentiation,ascites,CA125,and tumor cytoreductive surgery satisfaction,postoperative recurrence between low NLR group and high NLR group,low PLR group and high PLR group(P<0.05).The sensitivity and specificity of NLR,PLR combined with CA125 in the diagnosis of epithelial ovarian cancer were higher.≥50 years old,NLR≥2,PLR≥150,stage III-IV,G3,lymph node metastasis,satisfaction of tumor cytoreductive surgery were risk factors for prognosis.PLR,FIGO stage and tumor cytoreductive surgery satisfaction were independent risk factors for overall survival.Conclusion NLR,PLR combined with CA125 can be used as sensitive indicators for the diagnosis and prognosis of epithelial ovarian cancer.High NLR and high PLR indicate poor prognosis.
