摘要
目的观察依达拉奉联合尼莫地平治疗蛛网膜下腔出血的效果及对患者脑微循环、认知功能的影响。方法随机选取我院2018年1月至2019年1月收治的48例蛛网膜下腔出血患者为研究对象,按治疗方案将其分为对照组和观察组,各24例。对照组在常规治疗基础上给予尼莫地平治疗,观察组在对照组基础上联合依达拉奉治疗。比较两组患者的临床疗效、脑血管痉挛情况及治疗前、后的脑微循环指标、炎症因子水平及认知功能。结果观察组治疗总有效率高于对照组,脑血管痉挛总发生率低于对照组(P<0.05)。治疗后,两组颅内压及CRP、IL-6、TNF-α、S100β蛋白水平均降低,CBV、CBF均升高,且观察组优于对照组(P<0.05)。治疗后,观察组NIHSS、GCS评分均优于对照组(P<0.05)。结论依达拉奉联合尼莫地平治疗蛛网膜下腔出血的效果显著,可减少脑血管痉挛,改善脑微循环,控制炎症反应,缓解神经损伤,提高认知功能,值得临床应用。
Objective To observe the effect of edaravone combined with nimodipine in the treatment of subarachnoid hemorrhage and its influences on cerebral microcirculation and cognitive function of patients.Methods A total of 48 patients with subarachnoid hemorrhage admitted in our hospital from January 2018 to January 2019 were randomly selected as the research objects.According to the treatment plan,the patients were divided into control group and observation group,with 24 cases in each group.The control group was treated with nimodipine on the basis of conventional treatment,and the observation group was treated with edaravone on the basis of the control group.The clinical efficacy,cerebral vasospasm and cerebral microcirculation indexes,inflammatory factors levels,cognitive function before and after treatment of the two groups were compared.Results The total effective rate of treatment of the observation group was higher than that of the control group,and the total incidence of cerebral vasospasm was lower than that of the control group(P<0.05).After treatment,intracranial pressure and the levels of CRP,IL-6,TNF-αand S100βprotein of the two groups decreased,CBV and CBF increased,and those of the observation group were better than the control group(P<0.05).After treatment,NIHSS and GCS scores of the observation group were better than those of the control group(P<0.05).Conclusion Edaravone combined with nimodipine has a significant effect in the treatment of subarachnoid hemorrhage,it can reduce cerebral vasospasm,improve cerebral microcirculation,control inflammatory reaction,alleviate nerve injury and improve cognitive function,which is worthy of clinical application.
