摘要
目的:挖掘王行宽治疗冠心病心绞痛的遣方用药规律。方法:收集王行宽近3年来在湖南中医药大学第一附属医院门诊收治的冠心病心绞痛患者的医案186份。规范医案中出现的药味、处方,应用中医传承辅助平台(V2.5)对用药及经方、验方分别进行"药频分析""组方规律""新方分析"。结果:通过对筛选出的186首处方进行分析,用药频数统计前5位为麦冬、黄连、瓜蒌皮、法半夏、五味子;四气五味归经频数统计发现药性以温、寒为主,药味以苦、甘为主,归经多属心脾经;常用验方为生脉陷胸汤,喜用经方茯苓杏仁甘草汤;根据药物关联规则演化得到核心药物组合12组,潜在新处方6首。结论:王行宽治疗冠心病心绞痛善用经方及验方,常用方为茯苓杏仁甘草汤及生脉陷胸汤,用药方面常寒温并用,并善用药对,且用量平和。
Objective: To explore the regularity of WANG Xing-kuan’s prescription in treating angina pectoris.Methods: 186 cases of angina pectoris in The First Affiliated Hospital of Hunan University of Chinese Medicine in recent 3 years were collected. To standardize the taste and prescription of medicine in medical records, The Auxiliary Platform of Traditional Chinese Medicine Inheritance(V2.5) was used to carry out "medicine frequency analysis", "prescription rule" and "new prescription analysis" on medication, classical prescriptions and proved prescriptions respectively. Results: Through the analysis of 186 selected prescriptions, the top five prescriptions were Maidong(Ophiopogonis radix), Huanglian(Coptidis rhizoma), Gualoupi(Benincasae exocarpium), Fabanxia(Pinelliae rhizoma praeparatum) and Wuweizi(Schisandrae chinensis fructus). According to the statistics of frequency of channel tropism of four Qi and five flavors, it was found that the drug properties were mainly warm and cold, the drug tastes were mainly bitter and sweet, and the channel tropism mostly belonged to heart and spleen meridians. The commonly used prescription was Shengmai Xianxiong Decoction( 生脉陷胸汤), and the classic prescription was Fuling Xingwen Gancao Decoction(茯苓杏仁甘草汤). According to the drug association rules, 12 core drug combinations and 6 potential new prescriptions were obtained. Conclusion: WANG Xing-kuan is good at using classical and proven prescriptions in the treatment of angina pectoris, and the commonly used prescriptions are Fuling Xingwen Gancao Decoction and Shengmai Xianxiong Decoction. WANG Xing-kuan often uses cold and warm medicine, and is good at using paired drugs, and the dosage is mild.
