摘要
吕文良教授认为,肝癌之病机为本虚标实,其中正气亏虚为肝癌的病理基础,毒瘀互结则为其促发因素,且两者并行不悖,贯穿病程始终。因此在诊疗时始终以扶正祛邪为纲,并依病情所异而斟酌损益。临证时用生黄芪、炒白术、防风以增其实卫固表、扶正益气之功;山楂、麦芽、神曲化中焦食积瘀血之实,并助脾胃健运以补气血之虚;炒杏仁、姜厚朴、茯苓、法半夏、藿香、佩兰醒脾开胃,宣通气机,利湿祛痰;黄芩、黄连、白花蛇舌草、半枝莲、半边莲、熊胆粉清热解毒,消瘀散结;乳香、没药、芒硝活血散瘀,利气通络;白芍、甘草养血柔肝,补益肝体。
Professor Lv Wenliang believes that the pathogenesis of liver cancer lies in defi ciency of essence and excess of manifestation,of which the defi ciency of vital qi is its pathological basis,and intermingled toxin and blood stasis is the promoting factor,and the two go hand in hand throughout the course of the disease.Therefore,in the diagnosis and treatment of liver cancer,strengthening vital qi to eliminate pathogenic factors is upheld as a guiding principle,and profi t and loss should be considered according to diff erent conditions.In clinical practice,he is good at using unprocessed milkvetch root,stir-fried largehead atractylodes rhizome and divaricate saposhnikovia root to consolidate defensive exterior and strengthen vital qi;using hawthorn fruit,germinated barley,medicated leaven to dissolve food retention and blood stasis in middle jiao and help spleen and stomach to invigorate qi and blood circulation;using stir-fried bitter apricot seed,ginger magnolia,Indian bread,processed pinellia tuber,Agastache rugosa and fortune eupatorium herb to invigorate spleen qi and stimulate stomach,promoting qi movement,promoting diuresis and resolving phlegm;using baical skullcap root,golden thread,hedyotis,barbated skullcup herb,Chinese lobelia herb,bear gallbladder powder to clear away heat and detoxify,eliminating and dispersing blood stasis;using frankincense,myrrh and crystallized sodium sulfate to activate blood movement and disperse blood stasis,promoting qi movement and dredging collaterals;using debark peony root,liquorice root to nourish blood and soften liver,tonifying and nourishing liver.
